The Investigation Of Risk Factors Of Acute Coronary Syndromes Complicating With Heart Failure

Objective:to investigate the risk factors of acute coronary syndromes(ACS) patients on admission compalicated with heart failure (HF),to provide the clinical evidence for the occurrence and treatment of cardial failure.Methods:.480patients with acute coronary syndromes were collected,and150patients compalicated with HF, HF-related factors were screened and compared between two groups.Unitary logistic regression anaslyes were performed to detect the possible factors to HF.These factors were further analyed by stepwise regression method and multivariable logistic regression anaslyses.Results:.(1)Patients with HF were elder (P<0.05), and had significantly higher rates with history of HF,Ischemic stroke,T2DM,as well as Chronic obstructive pulmonary disease(COPD)(P<0.05).The level of heart rate、Fasting blood glucose, WBC,Fibrinogen, High sensitivity C-reactive protein, Troponin I peak, Triglyceride and Regional wall movement abnormalities were higher (P<0.05), The level of Systolic blood pressure, Diastolic blood pressure, Hemoglobin, Albumin, Glomerular filtration rate and LVEF value were lower(P<0.05); Based on compared with non-ST elevation ACS,the possibility of ST elevation ACS were higher.(2) There were significant differences among The degree of stenosis, Coronary lesion type、TIMI、Three-vessel disease between HF group and non-HF group based on CAG (P<0.05).(3)Single factors logistic regression showed that Previous HF,Ischemic stroke,T2DM, Chronic bronchitis,,Fibrinogen, Troponin T peak,Regional wall movement abnormalities, Systolic blood pressure, Diastolic blood pressure, Hemoglobin, Albumin, Glomerular filtration rate, LVEF value, Triglyceride, ACS type and TIMI were associated with cardial failure attack (OR>1.00, P<0.05).(4) Multivariable logistic regression anaslyses found that LVEF value (OR=2073, P<0.01) and COPD (OR=5.355, P<0.01) and T2DM (OR=2.015, P<0.05),Glomerular filtration rate (OR=1.015, P<0.05) were independent risk factors of HF.Conclusion:The possibility of ACS patients with HF was high which was impacted with various factors.Among the factors, LVEF value、COPD、GFR、T2DM are independent risk factors for HF in patients with ACS.
