Purpose:In this study, two restoration approach, all on4technologies and thetraditional telescopic crown implanted-overdenture, were discussed mainly throughthree-dimensional finite element analysis method, under loading stress. By observingthe stress changes of the superstructure of the implants, residual alveolar bone, andimplant itself, comparative analysis of the model was carried.Methods:To select an edentulous woman with integrity jaw and good health asresearch subject, lead the data got from a CT scan into the DICOM format, input it intothe analy software, and then add the implant, abutment and superstructure of theimplants, we can build two sets of three-dimensional finite element model. Through thetwo loading methods of the surface load of200N perpendicular to the occlusal surfaceof the tooth and the most distal end load of100N perpendicular to the occlusal surface,to make the stress of two sets change, then reading the value of the stress from thepalette generated by the catia, we did the three-dimensional finite elementcomparative analysis.Result:1: The superstructure of the implants:The maximum stress generated by allon4and telescopic crown implanted-overdenture model under the200N vertical loadon the occlusal surface is2.8MPa,4.2Mpa. The maximum stress generated by all on4isless than the stress generated by telescopic crown implanted-overdenture. The meanstress generated by all on4is higher than the telescopic crown implanted-overdentureand the stress distributed broad and uniform. The maximum stress generated by all on4and the telescopic crown implanted-overdenture model at the most distal end underthe100N vertical load on the occlusal surface is11.1MPa、10.4MPa. The maximumstress generated by all on4is slightly higher than the planting sleeve crown overdenture, and all on4has a stress concentration of11.1Mpa in the other bracket parts.2: Residualalveolar ridge:The maximum stress generated by all on4and the telescopic crownimplanted-overdenture model under the200N vertical load on the occlusal surface is3.3MPa,3.5MPa. The maximum stress generated by residual ridge of all on4and thetelescopic crown implanted-overdenture is slightly the same, and the stress of the wholemandible is distributed broad and uniform. The maximum stress generated by all on4and the telescopic crown implanted-overdenture model at the most distal end under the100N vertical load on the occlusal surface is6.5MPa,4.0MPa. The maximum stressgenerated by all on4is higher than the telescopic crown implanted-overdenture,3:implant itself:The maximum stress of all on4and the telescopic crownimplanted-overdenture model generated by the6implant abutments, under the200Nvertical load on the occlusal surface is12.5MPa,12.0MPa,11.6MPa,14.1MPa,17.5MPa,16.2MPa. The maximum stress generated by all on4is less than the stressgenerated by the telescopic crown implanted-overdenture. The mean stress generated byall on4is slimly the same as telescopic crown implanted-overdenture. The maximumstress of the6implant abutments under the100N vertical load at the most distal end is34.4MPa,4.3MPa,1.2MPa,0.4MPa,5.8MPa,2.6MPa. The maximum stress generatedby all on4is significantly higher than the telescopic crown implanted-overdenturemodel, and the mean stress generated by all on4is higher than the telescopic crownimplanted-overdenture model.Conclusion:Analyzing in the view of biomechanical, in the two loading modes, thestress distribution of the all on4fixed restoration and the telescopic crownimplanted-overdenture, through the contrast in three aspects of the superstructure of theimplants, residual alveolar bone, implant itself, the stress concentration has nosignificant stress concentration, and all the stress values are within the normal mandiblebear range. All on4fixed restoration can obtain better stress distribution effect, thelong-term effects can be guaranteed.
