Primary Sjogren’s Syndrome Secondary To Renal Tubular Acidosis And Two Cases Of Analysis

Objective: To summarize Clinical manifestations, diagnostic criteria, pathogenesis,immunological characteristics and treatment of primary Sjogren’s syndrome combinedrenal tubular acidosis and to conducive its early diagnosis.Methods: To analyze the clinical manifestations and laboratory andimmunological data of two patients with primary Sjogren’s syndrome secondary to renaltubular acidosis in this study.Results: The two patients were sent to hospital due to polydipsia, polyuria, intermittentfatigue, hypokalemia. Arterial blood gas analysis prompted metabolic acidosis, the urine wasabnormal alkaline urine, pH>5.5; antinuclear antibodies, anti-SSA, anti-SSB antibodies werepositive; Schirmer test was positive; lip gland biopsy showed that lymphocytes and plasmacells were diffused distribution around lip glands and ducts. Finally, primary Sjogren’ssyndrome and renal tubular acidosis was diagnosed. The prednisone was given to inhibitimmunological reaction, at the same time, famotidine was given to protect gastric mucosa,Caltrate was given to provent osteoporosis, potassium citrate was given to rectifyhypokalemia. After the patients were administrated these drugs, symptom of fatigue, drymouth and others obviously improved, and blood potassium was normal.Conclusions:1.The patient with unexplained hypokalemia should detect blood gasanalysis to check whether there is the acid-base imbalance.2.The patient with renal tubular acidosis and unexplained hyperglobulinaemiashould be detected immunological indexes, also be done the lip biopsy at prorenate.which can help to identify primary drying syndrome or other autoimmune diseases, inorder to avoid misdiagnosis or missed diagnosis.
