【Objective】To explore the clinical manifestation and imaging of LMS,to deepenunderstanding of this syndrome,aim to provid the basis for clinical dignosis.【Methods】We analyzed clinical datas retrospectively of62patients with LMS,divi-ded the cases into three group(rostral、middle、caudal) according to skull MRI.Wemade a comparison of gender、age、number of ami days、risk factors、clinical manife-station,and then analyzed their characteristics of skull MRI and angiograms.【Results】The rostral lesions were usually diagonal band-shaped and deep,whereasthe caudal lesions situated in the lateral surface of the medulla.No significant werefound in gender、age、number of ami days、risk factors、nystagmus、nausea/vomiting、Horner sign、 sensory disfunction(ipsilateral trigeminal-contralateral limb/body、contralateral trigeminal-contralateral limb/body、 bilateral trigeminal-contralaterallimb/body)、hiccups、headache、diplopia、gaze among three groups(P>0.05).Rostraland middle groups were associated with more dysphagia、 nasal feeding andhoarseness than caudal group,and more facial paresis was found in rostral type thancaudal group(P<0.0125).Caudal group were associated with more vertigo/dizziness、gait ataxia than rostral group(P<0.0125).Middle and caudal types appeared tocorrelate with more isolated limb/body sensory disfunction than rostal type(P<0.0125).There were no diference between rostral and middle groups in dysphagia、nasal feeding、hoarseness,and no difference between middle and caudal groups inisolated limb/body sensory disfunction(P>0.0125).There were possibly more VAdiseases than PICA diseases in LMS according to angiograms.【Conclusion】The clinical manifestations of LMS are complex and varied,and areobviously discriminating among different lesion locations.Rostral and middle groupsare associated with more dysphagia、hoarseness than caudal group,and more facialparesis is found in rostral group.Caudal group are associated with more vertigo/dizzi-ness、gait ataxia than rostral group.Middle and caudal types appear to correlate with more isolated limb/body sensory disfunction than rostal group.The rostral lesions areusually diagonal band-shaped and deep,whereas the caudal lesions situate in thelateral surface of the medulla.There are possibly more VA diseases than PICA diseasesin LMS.MRI and angiography(especially the DSA) are indispensible in the dignosisan research of LMS.
