Objective: To explore the imaging technology of the64-slice spiral CT (64-MSCT),analyze the image characteristics of esophageal and gastric cancer,and evaluate its clinical applications.Methods:412cases with esophageal and gastric cancer (esophageal cancer in120, gastric cancer in292cases) were selected in our hospital from2006.3to2012.9. All patients were scanned with64-MSCT and had thesurgical and pathological confirmation. CT scan volume data were usedto have the image post-processing technologies, which are the volumeimaging (VR), multiplanar reformation (MPR), maximum intensityprojection (MIP)、 minmum intensity projection CT virtualendoscopy(CTVE), CT angiography (CTA), CT stomach imaging(CTG) andCTA technologies. The display rate of the lesion (including thesurrounding lymph nodes) and diagnostic accuracy were calculated.Results:64-MSCT displayed the esophageal lesions and surrounding lymph nodesvery well, the diagnosis accuracy of120patients with esophagealcancer is88.70%. The number of lymph node metastasis of120patientswith esophageal cancer in the upper(28cases), middle(64cases)and lower (including abdominal segment28cases) was46、130、54golds, with a total of230hgolds.204golds correctly diagnosed,30misdiagnosed and26weremissed by64-MSCT,64-MSCT diagnosis rateof lymph node metastasis metastatic lymph nodessensitivity is88.7%,specificity rate of96.7%, accuracy rate of95.1%, rate ofmisdiagnosis11.3%,misdiagnosis rate of3.3%. Stomach reconstructedimage multi-angle clearly show that the thickening of the stomachwall varying degrees or cavity bulge, irregular cavity surface uneven. The detection rate of axial image tumors in292cases of gastriccancer is81.85%(239/292), the MPR image detection rate being92.81%(271/292), two images combineddetection rate being94.18%(275/292).Virtual endoscopy of the140cases in the image lesion detection rateof90.71%(127/140), the lesions showed clear;84cases of gastricenhanced the judge TMN staging patients scanning combined withpost-processing techniques such as VR, MPR, MIP, MinP, accurate depthevaluation of tumor gastric wall infiltration rate of78.57%(66/84)、accurate lymph node metastasis rate of76.19%(64/84), andaccuratea distant metastasisrate of87.5%(28/32).The rate of68patients by CTA with lesions showed is80.88%(55/68),assessvascularanatomy diagnosis rate being97.06%(66/68). CTA of68patients show the lesions with the80.88%(55/68)and the vascularanatomy with the97.06%((66/68)).Conclusion:Used the64-MSCT scanning data, three-dimensional CT imagingtechniques can clearly display the size and location of the esophagusand gastric cancer, including the external invasion from the lesionand regional lymph node metastasis.64-MSCT diagnosis has a highcoincidence rate with that of the surgery and pathology. It can providean important basis for clinical diagnosis and treatment options.
