【Objectives】1.Study the survival and changes in liver function in rats with partial hepatct-omy by completely portal vein artery.2.When the rats with partial hepatectomy accepted the completely portal veinartery,study the changes of protein(prealbumin,albumin,et al.) and the amino acidprofile in the serum;and then to explore the effect for the protein synthesis of the liverby portal vein artery (PVA),and to guide the clinical applications of PVA.【Methods】1.Based on the method of cuff to establish the rat model of portal veinarterialization.Surgical methods:semi-opened anesthesia by ethyl ether, abdominalincision, separation of the left renal artery and vein,resect of the left kidney,and letthe allograft material set into the left renal artery and suture ends, connect the cuff oneend of the portal vein proximal to set into the ligation, the left renal vein with the cuffand the superior mesenteric venous connection.On this basis, remove the left lateraland nipple leaf of rat liver, accounting for about40%of the rat liver, and then theanimal model is successful.Grouping:A is30rats for PVA and removing the40%ofliver and left kidney;B is20for removing the40%of liver and left kidney;C is onlyfor removing left kidney.The survival rate,liver function,regeneration and pathologywere observed after surgery.2.Record the time of surgery, portal vein occlusion time, the survival rate forgroup A,Band C.After operation1d,3d,5d,7d,and10d,detect the content of albumin(ALB), prealbumin (PA) and total protein (TP). After operation3d an7d,detect theamino acid profile.【Results】1.Establish the rat model of portal vein arterialization and removing the40%of liver1.1The model success rate is90%(27/30), and the rat were all alive.One week after surgery the rats’weight change was not statistically significant (P>0.05). At eachtime point compared with each other, there was no statistically significant differencein ALP(P>0.05);On the third day after operation, AST and ALT in group A werelower than in group B and C,the difference was statistically significant (P <0.05); andCHE was higher, the difference was statistically significant (P <0.05), too.Thesesuggested that PVA has a significant role in the early stage of liver function ’srestoration.After1week, ALT in group A is lower than in group B and C,thedifference was statistically significant (P <0.05), prompted that PVA is still importantin promoting the recovery of liver function;the rest compared with each other therewere no significant difference (P>0.05). After1month, also showed no significantdifference (P>0.05).1.2On the third day and one week, the LRR in group A than in group B weresignificantly increased, the difference was statistically significant (P <0.05); after onemonth, the LRR in group A is also higher than in group B, but the difference was notstatistically significant(P>0.05). These deemed that PVA is conducive to the recoveryof liver function after partial hepatectomy in early time.1.3After one month,three groups of rats pathological findings showed noobvious abnormalities.2.The effect for the protein synthesis of the liver by the rat model of portalvein arterialization and removing the40%of liver2.1General:The surgical success rate of group A is93.3%(28/30),and100%ingroup B and C;three groups of rat portal vein occlusion time were:(16±2.5) min,15min,15min;the mean operative time were:(59±5.5) min,(40±4.5) min,(26±1.5)min.2.2Protein: postoperative1,3,5day, group A and B’s prealbumin (PA)compared with group C has significantly decreased,and the difference has statisticallysignificant (P <0.05),and on postoperative5day, prealbumin (PA)in group Acompared with group B has increased highly;total protein (TP) compared with groupC decreased highly.All of the difference have statistically significant (P <0.05).ALBin each group was no significant difference(P>0.05).2.3The amino acid profile in the serum:Ⅰ:3days after surgery, Ala,Tyr,Phe,Lysin group A and B compared with group C increasedhighly;Val,Ile,Leu,Arg,Trp,BCAA/AAA in group A and B compared with group C decreased highly,all of the difference have statistically significant (P <0.05).Group Acompared with group B,Val decreased highly;Leu and Lys increased highly, and thedifference has statistically significant (P <0.05);compared with group C,Gly and Hisdecreased,Met increased,but the difference has no statisticallysignificant(P>0.05).Ⅱ:7days after surgery,Ile,Leu,Arg,Trp,BCAA/AAA in group Acompared with group B increased highly,Tyr,Phe decreased highly;compared withgroup C,Ala,Lys increased highly,all of the difference have statistically significant (P<0.05).Ala,Tyr,Phe,Lys in group B compared with group C increasedhighly;Ile,Leu,Arg,Trp,BCAA/AAA decreased highly,all of the difference havestatistically significant (P <0.05).The other amino acids raise or decrease,but thedifference compared with each other were not statistically significant (P>0.05).【Conclusions】1.The rat model of portal vein arterialization,which established by theallogeneic artery and cuff technique and had a high portal vein patency,was afeasible,stable,simple and reproducible model.2.The portal vein arterialization can promote liver regeneration, restore liverfunction in the short-term, and experimental animals can live.3. The portal vein arterialization can enhance protein synthesis and recovery ofplasma protein levels after partial hepatectomy in the early time。4.The portal vein arterialization can effectively enhance BCAA/AAA ratio,recover serum amino acid levels more rapidly than in the group of non-arterial inshort term, and can enhance the effect for amino acid metabolism.
