Abstract/Summary:
ObjectiveComparison of early recurrence after hepatic resection in patients with differenthepatitis virus-related hepatocellular carcinoma (HCC) and explore their potentialeffecting factors, datum of virus inspection, clinical and follow-up visit were studiedin HCC patients confirmed by pathology.MethodsThe study population consisted of160patients (7have lost follow-up visit)undergoing a curative resection at The First Affillated Hospital of Fujian MedicalUniversity for HCC confirmed by pathology from November2008to January2012.The patients were divided into four groups: neither HBV nor HCV (NBNC) groups,HBV only groups, HCV only groups and HBV and HCV co-infected groupsaccording to the presence of HBV and HCV which determined by serum inspection,to increase the efficacy and specificity of the HCV detection nested reversetranscription polymerase chain reaction (nRT-PCR) were also carried out inhepatocellular carcinoma tissue. At last we explore the role of HBV and HCV in theearly recurrence and metastasis of postoperation HCC and explore their potentialeffecting factors by retrospective analysising their clinicpathologic characteristics.Results1. nRT-PCR is an advantage method to detect the hepatitis C patients whoseserum ELISA testes were HCV negative. HCV was found in39of160patients usenRT-PCR (24.38%), while infection by HCV only was found in12patients detectedby serum ELISA testes (7.5%)(p<0.05). 2. Among the follow-up cases, there were44patients with postoperativerecurrence, and the recurrence rate is28.76%. The postoperative recurrence case ofneither HBV nor HCV (NBNC) groups, HBV only groups, HCV only groups andHBV and HCV co-infected groups were1,28,5and10respectively, the recurrencerate was5%,29.79%,27.78%,47.62%respectively. Meanwhile the cases with HBVor (and) HCV positive seems have higher cumulative recurrence rate compared withthe NBNC groups, HBV and HCV co-infected groups seems have higher cumulativerecurrence rate compared with the HBV or HCV only groups (P<0.05),but there isno difference between HBV only groups and HCV only groups.3. There are significant difference among age, preoperative AFP, preoperative PT,postoperative total bilirubin, tumor size, the extent of cirrhosis and the degree oftumor differentiation between each groups (P<0.05), and the extent of cirrhosis andpostoperative total bilirubin are significant higher in patients with HBV infected(included HBV and HCV co-infected);the degree of tumor differentiation issignificant lower in patients with HCV infected (included HBV and HCVco-infected);the preoperative AFP, tumor size of patients with HBV and HCVco-infected are significant higher or larger (P<0.05).Conclusion1. nRT-PCR is an advantage method to detect the hepatitis C infected.2. HCC patients with HBV infection (include HBV and HCV co-infection) havea larger degree of cirrhosis and higher billirubin level after hepatic resection, andhigher postoperative early recurrence rate compared with the noninfection.3. HCC patients with HCV infection (include HBV and HCV co-infection) havea lower degree of tumor differentiation, and higher postoperative early recurrenceratecompared with the noninfection.
