【Objective】To investigate the independent risk factor in perihematoma edemafollowing hypertensive intracerebral hemorrhage, and to further explore the impact ofblood pressure variability in it.【Methods】This was a retrospective analysis of study of87inpatients withhypertensive intracerebral hemorrhage,from May2006to November2012.The basicinformation, blood pressure value, intervention and CT images of these inpatientswere collected,hematoma volumes and total lesions volumes were calculated fromCT images.Putting the whole data in order,Blood pressure variability indicated daysvariability and was quantified by calculating the standard deviation(SD)、coefficientof variation(CV).For these variability might positively correlated with mean ofblood pressure,the additional variability measure,SD independent of mean(SDIM),was also calculated. The expand of perihematoma edema was defined when totallesions value T2was larger then total lesions value T1.Logistic regression analysiswas performed in the data of the blood pressure variability indexs,mean,course of thedisease,drugs and so on,using backward stepwise regression to access the independentrisk factors of perihematoma edema.【Results】By Logistic regression analysis,SBP-CV(F=6.909,P=0.009)、SBP-SD(F=8.401,P=0.004)、SBP-mean(F=6.625,P=0.01)、Amlodipine(F=9.733,P=0.002)were statistically significant.Amlodipine(OR=0.161,95%CI=0.051~0.507)was protective factor of perihematoma edema,and SBP-CV (OR=13.992,95%CI=1.956~100.08)may be risk factor.There was no correlation betweensex,Age,mannitol,glycerol fructose,bumetanide, RAS classes of antihypertensiveagents,SBP-SDIM,DBP-SD,DBP-CV,DBP-mean,course of the disease,hematoma volume and perihematoma edema.【Conclusion】 The increasing of mean of systolic blood pressure are the strongpredictive value of risk factors on perihematoma edema following hypertensiveintracerebral hemorrhage. Systolic blood pressure variability may have a badinfluence on perihematoma edema following hypertensive intracerebral hemorrhage,but not the determining factor, but on the basis of mean of systolic blood pressure.Amlodipine can lower the risk of perihematoma edema following hypertensiveintracerebral hemorrhage. Rational using of antihypertensive agents and controling ofsystolic blood pressure levels,which was likely to reduce the risk of expand ofperihematoma edema.
