BackgroundWith the wide use of broad-spectrum antibiotics clinically, bacterial drug resistance phenomenon is increasingly serious. Pseudomonas aeruginosa (PA) infection rate and fatality rate caused by PA infection are still at the high level. The sensitivity of pseudomonas aeruginosa to ultra broad spectrum antibiotics is declining. The mechanism of pseudomonas aeruginosa resistance is very complicated, mainly including inactivated enzyme production, changes in drug action, lower permeability of outer membrane and drug to reduce the flow of active efflux pump, etc. Multi-resistant became a serious problem in pseudomonas aeruginosa infection. Especially in recent years appeared pandrug-resistant pseudomonas aeruginosa(PDRPA),that resistant to all kinds of antibiotic resistance to pseudomonas except polymyxin. Polymyxin is the currently available antibiotics on the market. It would often make the clinician helpless to high fatality rate and serious clinical consequences, once a patient has been infected. Traditional Chinese medicine in inhibiting bacterial resistance has made some achievements, mainly on the research of single herbs and traditional Chinese medicine monomer, and TCM compound also have positive effects on drug-resistant bacteria in vivo and in vitro experimental report. According to the clinical observation, drug-resistant bacteria infection corresponds to the TCM Fu-Xie pathogenesis. Treating warm disease caused by Fu-Xie, do not overuse the method with bitter and cold herbs, but the method of clearing heat and expelling pathogenic. According to the method we establish the treatment of drug-resistant baeteria infection that combining Xin Jia Da Yuan San with antibiotic which obtaining good results in clinical. Therefore we put forward the hypothesis that the method of clearing heat and expelling pathogenic based on the theory of Fu-Xic can inhibit/reverse bacterial resistanceObjectiveStudy the role of inhibitting/reversing antibiotic resistance by Xin Jia Da Yuan San. MethodsSerum pharmacology method is applied to prepare drug serum to simulate the actual compounds after oral medicine absorbed. Experiment1cultured the pseudomonas aeruginosa in broth medium with drug serum and low concentration levofloxacin. Blank control group using rats blank scrum treatment, at the same time setting up a group to culture PA in broth medium with simple antibiotics without serum to remove the interference factor in serum itself. Subsequent subculture, observed each generation culture of bacteria, until a group of bacteria becoming drug resistance. Kirby-Bauer method was used to measure the antibiotic sensitivity and resistance of bacteria. And measured each MTC of the group. Experiment2cultured resistance pseudomonas aeruginosa in broth medium with drug serum mixed, blank control group using rats blank serum treatment, at the same time setting up a group to culture bacteria in broth medium without serum to remove the intorference factor in serum itself. Subsequent subculture, observed each generation of culture of bacteria, until a group of drug-resistance bacteria reversed obviously. Kirby-Bauer method was used to measure the antibiotic sensitivity and resistance of bacteria. And measured each MIC of the group.ResultsEexperiment1, the FA ATCC27853cultured in drug serum group became drug resistance later than in blank serum group and blank control group, with statistical difference (P<0.01). Eexperiment2, the PA25029cultured under the effect of drug serum intervented, to the ninth generation the inhibition zone of levofloxacin was (15.37mm±0.58) in diameter, according to CLSI standard interpretation had been restored to the intermediate level,compared with the b1ank serum group and b1ank control group had significant changes (P<0.01). Cultured to twelfth generations later, pseudomonas aeruginosa25029kept sensitive to the levof loxacin(inhibition zone in diameter>17.00mm).ConclusionIn vitro, the method of c1earing heat and expelling pathogenic (Xin Jia Da Yuan San serum) may delay or inhibit the revsistance in pseudomonas aeruginosa to levof1oxac in and reverse the resistance to levofloxacin.
