1. ObjectiveIn order to clear the clinical efficacy of green tea polyphenols capsule combined with chemotherapy in advanced non-small cell lung cancer and evaluate its possible mechanism of inhibition of lung cancer angiogenesis by a randomized controlled clinical study2. MethodsNon-small cell lung cancer patients of stage Ⅲb and Ⅳ as the research objects were randomly divided into three groups after the Endostar combined with chemotherapy group selected the patients who could afford the medical expenses.116effective cases undergoing conventional chemotherapy were collected with simple chemotherapy group32cases, tea polyphenol capsule combined with chemotherapy group30cases taking2tea polyphenols capsules orally2times a day at the same time, Shen Yi capsule combined with chemotherapy group30cases taking2Shen Yi capsules orally2times a day at the same time, Endostar combined with chemotherapy group24cases were given Endostar injection7.5mg/m2by intravenous infusion once every day for14days,21days again. The efficacy was evaluated after4weeks, evaluation indexes include the effective rate, clinical benefit rate, TCM symptom score, KPS score, tumor markers(Cyfra211,CEA,NSE), immune function (CD4+/CD8+, FOXP3), neovascularization associated factor (VEGF,TIMP-2) expression level.3. Results①effective rate, clinical benefit rate:the effective rate and clinical benefit rate between four groups didn’t show statistical difference.②clinical symptom efficacy:The mean score descent from pre-treatment to post-treatment between tea polyphenols capsule combined with chemotherapy group and Shen Yi capsule combined with chemotherapy group had statistical significance (P<0.05). For improving symptom, tea polyphenols capsule combined with chemotherapy group and Shen Yi capsule combined with chemotherapy group were better than the other two groups (P<0.01),but the difference after treatment between them was not statistically significant (P>0.05).the same as simple chemotherapy group and Endostar combined with chemotherapy group (P>0.05). It shows that tea polyphenols capsule and Shen Yi capsule can improve symptoms obviously.(3)KPS score:the four groups’differences of KPS score between pre-treatment and post-treatment were not statistically significant (P>0.05). tumor markers(Cyfra211,CEA,NSE):the four groups’differences of every tumor marker between pre-treatment and post-treatment were not statistically significant (P>0.05).⑤immune function (CD4+/CD8+, FOXP3):the rise of CD4+/CD8+and the descent of FOXP3after treatment of tea polyphenols capsule combined with chemotherapy group and Shen Yi capsule combined with chemotherapy group were statistically significant (P<0.05). The descent of CD4+/CD8+in simple chemotherapy group from pre-treatment to post-treatment was statistically significant (P<0.05), but that of Endostar combined with chemotherapy group was not statistically significant (P>0.05). The difference of FOXP3from pre-treatment to post-treatment in the simple chemotherapy group and Endostar combined with chemotherapy group were not statistically significant (P>0.05).The functions of increasing CD4+/CD8+and decreasing FOXP3in tea polyphenols capsule combined with chemotherapy group and Shen Yi capsule combined with chemotherapy group were better in statistics than the other two groups (P<0.05).The difference of CD4+/CD8+and FOXP3after treatment between tea polyphenols capsule combined with chemotherapy group and Shen Yi capsule combined with chemotherapy group were not statistically significant (P>0.05), the same as the simple chemotherapy group and Endostar combined with chemotherapy group (P>0.05). It shows that tea polyphenols capsule and Shen Yi capsule can up-regulate CD4+/CD8+, down-regulate the expression of FOXP3, improve patients’ immune function,⑥neovascularization associated factors:The mean descent of VEGF from pre-treatment to post-treatment of every group was statistically significant (P<0.01), but the mean rise of TIMP-2was not(P>0.05). For the function of decreasing VEGF, Endostar combined with chemotherapy group was best in statistics(P<0.01or0.05), tea polyphenols capsule combined with chemotherapy group and Shen Yi capsule combined with chemotherapy group were better than the simple chemotherapy group in statistics(P<0.05),but there was no difference between them in statistics (P>0.05).The difference of TIMP-2after treatment among four groups was not statistically significant(P>0.05). It shows that for the function of decreasing VEGF in this study category, Endostar combined with chemotherapy group>tea polyphenols capsule combined with chemotherapy group, Shen Yi capsule combined with chemotherapy group> simple chemotherapy group. Four groups all have the trend to increasing TIMP-2.The differences of VEGF and TIMP-2in pre-treatment and post-treatment between the patients of stage Illb and Ⅳ were not statistically significant (P>0.05).The decent of VEGF from pre-treatment to post-treatment of the patients of stage Ⅲb and Ⅳ was statistically significant (P<0.05). The rise of TIMP-2from pre-treatment to post-treatment, patients of stage Illb was statistically significant (P<0.05), patients of stage IV were not(P>0.05). It shows that the changes of VEGF and TIMP-2were independent of the clinical stage of Ⅲb and Ⅳ, but the patients of stage Ⅲb’rise of TIMP-2were evident in statistics. The differences of VEGF and TIMP-2in pre-treatment and post-treatment between the patients with effectiveness and without effectiveness were not statistically significant (P>0.05). The decent of VEGF from pre-treatment to post-treatment of the patients with effectiveness and without effectiveness was statistically significant (P<0.05), but the rise of TIMP-2was not (P>0.05). It shows that the changes of VEGF and TIMP-2were independent of the curative effect.4.ConclutionsTea polyphenols capsule combined with chemotherapy group and Shen Yi capsule combined with chemotherapy group can improve symptoms obviously, up-regulate CD4/8, down-regulate the expression of FOXP3, improve patients’ immune function; For the function of down-regulating the expression of VEGF in this study category, Endostar combined with chemotherapy group> Tea polyphenols capsule combined with chemotherapy group and Shen Yi capsule combined with chemotherapy group> simple chemotherapy group. For the function of anti-angiogenesis, Endostar is best, tea polyphenols capsule and Shen Yi capsule are at the same level. patients in stage Ⅲb could better up-regulate the expression of TIMP-2...
