Purpose:1、 To research the application of24h ambulatory blood pressure monitoring inthe evaluation of essential hypertension patients′target organ damage.2、 Try to explain the hypertension patients′blood pressure changes and targetorgan damage, using the traditional Chinese medicine theory.Material and method:The research object is496patients with essentialhypertension as the first-listed diagnosis, who are selected from the hospital-lized patients and outpatients of the internal medicine-cardiovascular depa-rtment in our hospital between August2010and April2012.According to themedical history and physical examination, we exclude the patients who are suf-fering from secondary hypertension or primary heart、brain or kidney diseases.The496patients are picked out to perform ABPM and related target organ exa-minations. According to the condition with or without target organ damage, theyare divided into group A:pure hypertension group(hypertension group)(n=215);group B: hypertension and artery atherosclerosis group(artery atherosclerosisgroup)(n=24);group C: hypertension and heart damaged group(heart damaged gro-up)(n=73); group D: hypertension and brain damaged group(brain damaged group)(n=88); group E: hypertension and kidney damaged group(kidney damaged group)(n=25); group F: hypertension and many target organs damaged group(many targetorgans damaged group)(n=71).The ABPM parameters are valuated with statisticmethods.Results:1、 The blood pressure level of target organ damaged groups is higher than purehypertension group, and the level of the systolic blood pressure and nocturnalblood pressure is obviously higher.2、 The incidence of target organ damage in abnormal circadian rhythm group is higher than dippers rhythm group′s.3、 The pulse pressure and blood pressure load of target organ damaged groupsare larger than pure hypertension group′s, and the systolic blood pressureof heart damaged group is obviously larger, the circadian blood pressure loadof brain damaged group is continually larger.4、 The result of the comparison between the blood pressure of TCM syndrome typesis Hyperactivity of liver-yang﹤Hyperactivity of yang due to yin deficiency=Stagnation of phlegm-dampness﹤Deficiency of yin and yang.5、 Abnormal circadian rhythm of Hyperactivity of yang due to yin deficiency、Deficiency of yin and yang and Stagnation of phlegm-dampness is more thanHyperactivity of liver-yang′s. Two kinds of rhythm in Hyperactivity ofliver-yang have no obvious difference.6、 The incidence of target organ damage in Stagnation of phlegm-dampness andDeficiency of yin and yang is higher than Hyperactivity of liver-yang′s.Conclusion:ABPM can accurately reflect the blood pressure of hypertensionpatients, and has a strong correlation with hypertension target organ damage.Hypertension target organ damage is actually organs′blood supply and demandimbalances caused by vascular damage. Using the combination of traditionalChinese and Western medicine treatment, we can regain the balances of organs′blood supply and demand, and recovery the adjustment function. The comprehensiveeffect is to improve the quality of the patients′life.
