ObjectiveWe aim to investigate the relationship between the grading or staging of the liver histopathology and the age, ERα or AR in patients who suffering from chronic hepatitis B, and by measuring T cell receptor rearrangment excision circles (TREC) content of the peripheral blood mononuclear cells, we study the effect of thymus function that changing with age in the progression of chronic hepatitis B.MethodWith reference to the diagnostic criteria of the"Guideline on prevention and treatment of chronic hepatitis B in China (2010)", we selected123cases of hepatitis B patients with liver biopsy, the number of males was83, and the females was43, age of the patients was among12-75years old. One week before and after liver biopsy,5ml venous blood was obtained from the patients, Ficoll density gradient centrifugation to obtain PBMC, then add300μl RNA later to the cell after centrifugation, and freezing the cells at minus80degrees Celsius refrigerator. Estrogen and androgen receptor mRNA in PBMC measured by real-time RT-PCR.We measured T-cell receptor rearrangement excision circles in PBMC using real-time quantitative PCR method. We used one second percutaneous transhepatic method to extract liver tissue, the liver tissue pathological gradings and stagings were determined after reading by authority pathologist. Estrogen and androgen receptor of liver tissue were measured by immunohistochemistry.ResultsPart11.The expression of ERα in the liver tissue of patients with chronic hepatitis B is not different between different genders and pathological staging, but difference existed among different pathological grades, P=0.008;2.The ERα mRNA level of PBMC was not different between men and women,and did not correlated with liver histopathological grading, but it had difference among different staging of patients with hepatitis B, rs=-0.213, P=0.018;3.The AR mRNA level of PBMC had no difference between men and women, but it was negatively correlated with age, r=-0.317. the level of AR mRNA was different between different grading or staging of the liver tissue;4.There is a positive correlation between age and liver pathological staging, rs=0.263. Part21. The TREC level in PBMC had no difference between different liver tissue grading or staging level;2. TREC in PBMC is negatively correlated with age, the thymus recent output function is decreasing gradually with age, rs=-0.298;3. TREC in PBMC show sex difference,P=0.049,TREC content in female patients is higher than men.Conclusion1. The level of ARmRNA in PBMC have some predict effect of the liver pathological grading and staging, this give a new non-invasive assessment of liver inflammation activity and fibrosisbasis; ERa have certain impact on the chronic liver inflammation extent in patients who suffering from chronic hepatitis B;2. TREC in PBMC relate with the age and sex of the patients suffering from chronic hepatitis B,The sex difference of the incidence of chronic hepatitis B perhaps result from the difference of TREC between men and women.
