It is widely accepted that the provision of safe quality healthcare depends amongstother things, on the availability of reliable and high quality patient information at thepoint of care. The fragmented nature of healthcare systems is one of the majorchallenges in the healthcare informatics domain. To address this problem, standards andtechnical specifications are being developed with the aim to structure the clinicalcontent for the purpose of exchange with the goal of providing interoperable solutions.The primary objective of this research was to develop a set of content specificationfor sharing electronic health record (EHR) across organizations. The study took the“Basic dataset oF heaIth record for residents”“HeaIth data element dictionary” and“Classification and coding for vaIue domain of health data element” as the starting point,propose a method to marry local data element and data set standards with internationalstandard HL7CDA R2, The main contents and results of this study include thefollowing:1. Initially the importance of the local data element and data set standards wasidentified and its importance in relation to health information exchange.2. The structures of popular international health information exchange standards areanalysed. Various case studies were identified which provided an insight tointernational adoption and implementations of the CDA.3. The benifits of templated-CDA and CDA’s “incremental interoperability” strategyare analysed, based on the approach taken by standard organizations such as HITSPIHE HL7and countries such as Australia and Germany, data groups as reusablebuilding blocks for “Basic dataset oF heaIth record for residents” were developed,and corresponding to each data group a CDA template was defined throughimplementation guide, schematron files are added.4. Based on the GreenCDA method a simple XML format for the representation ofheaIth record for residents was designed, and the XSLT files were mannuallydesigned to transform either from canonical XML to simplifed XML or vice. 5. UML domain models for CDA implementation guide for EHR were establishedusing MDHT modeling tools, then Java API were generated for creating,parsingand valdating CDA documents through these UML domain models.Through the application of this study, it is easy to structure the representation ofclinical documents for the exchange and sharing of data between disparate healthcareinformation systems, facilitate the adoption of international standards HL7CDA R2andimplementation during the development of interfaces between systems.
