The growth of incidence and mortality of lung cancer year by year badly threatens people’s health. The key to prolong survival period and lower the mortality of patients with lung cancer is early diagnosis and effective treatment. At the moment the routine diagnostic method of western medicine, including fibrotic bronchoscopy, radiographic inspection, CT, MRI, determination of serum tumor markers, sputum cytology, percutaneous lung puncture biopsy and etc., is able to make early diagnosis of lung cancer, yet which is complicated, invasive, and expensive. The traditional Chinese medicine (TCM) focusing on integral view and governing exterior to infer interior, can achieve an early diagnosis and timely treatment. Therefore, the observation and conclusion of specific diagnostic methods of TCM probably makes a contribution to early diagnosis of lung cancer. The present researches of TCM diagnostic specificity in Chinese medicine mainly fasten on acupoint diagnosis, ear diagnosis, tongue diagnosis, and nail diagnosis, hand diagnosis and facial diagnosis.This essay chose an early lung cancer diagnosis standard as the set of standard after analysis of pertinent literature, observed the positive reaction of acupoints, ear acupoints, hand diagnosis and facial diagnosis of the subjects including120cases of early primary lung cancer patients,60cases of benign lung disease patients and60cases of healthy people, inductively analyzed some clinical syndromes, signs of early lung cancer, protocolled identification points of TCM diagnostic specificity of early primary lung cancer and some dependency between clinical signs and early lung cancer diagnosis, and lastly provide preference for further researches of TCM diagnostic specificity of early lung cancer.Subjects:120cases of early primary lung cancer patients(all of them before the IIB period and without operation and chemotherapy) from thoracic surgery inpatient department in Districts1in Xuanwu Hospital from2011June to October,60cases of pulmonary non-neoplastic disease patients from respiratory disease inpatient department in Guanganmen hospital from2011November to December and60cases of healthy people are observed and recorded positive reaction (including tenderness and tubercle) of acupoints, ear acupoints, tongue, hand diagnosis and facial diagnosis in the study.Methods:First, according to analysis in early lung cancer diagnostic standard stated in pertinent literature and supervisor’s long-term clinical experience, subjects were divided into3groups, which were early lung cancer group, pulmonary non-neoplastic disease group and healthy group respectively.Secondly, people who meet the set of standard were chosen and fit into early lung cancer group, pulmonary non-neoplastic disease group and healthy group respectively.Third, the clinical syndromes and signs of listed in early lung cancer diagnostic standard in pertinent literature were reorganized and18indicators with high frequency were selected as index of clinical observation and they were lung (left ear) tubercle, lung (left ear) tenderness, lung (right ear) tubercle, lung (right ear) tenderness, lung-shu (left) tubercle, lung-shu (left) tenderness, lung-shu (right) tubercle, lung-shu (right) tenderness, new Daxi point (left) tenderness, new Daxi point (right) tenderness, vertical stripes of Line-I(left hand), vertical stripes of Line-I(right hand), zygotic erythema(left), zygotic erythema (right), pink tongue, thin and whitish fur, bright sublingual vessel, dorsal hickie.Forth, basic clinical features of subjects in3groups including age, gender, the primary tumor site, pathological type, clinical stages, KPS scores, history of smoking, tumor family history and etc.Lastly, statistical analysis of the observed signs testified whether there was dependency between the clinical signs and early lung cancer the diagnostic significance of the clinical signs.Results:First, there was no statistically significant difference in data of gender and tumor family history in all3groups; the primary tumor site, pathological type, clinical stages as the early lung cancer group’s specific clinical features was not statistically significant; the KPS scores of early lung cancer group was the highest while the lowest was healthy group; patients of both lung cancer group and pulmonary non-neoplastic disease group had higher rate of smoking and longer smoking history than healthy group. Second, clinical positive signs of early lung cancer group and pulmonary non-neoplastic disease group were concluded through the analysis of statistical data and thus the indexes of TCM diagnostic specificity was able to be quantized. The main different indexes of early lung cancer group and healthy group were new Daxi point (left) tenderness, lung shu point (left) tenderness and new Daxi point (right) tenderness and the secondary indexes were lung (left ear) tubercle and lung (right ear). The main different indexes of early lung cancer and pulmonary non-neoplastic disease group were new Daxi point (left) tenderness> pink tongue, thin and whitish fur. The main different indexes of pulmonary non-neoplastic disease group and healthy group were lung-shu (left) tenderness, lung-shu (right) tenderness, bright sublingual vessel, lung (right ear) tubercle and thin and whitish fur.Conclusion:There was no statistically significant difference of data of gender in3groups in the study. Meanwhile patients of early lung cancer were the oldest, with the highest rate of smoking and the longest smoking history comparing with the other two groups.The study had identified the dependent indexes of clinical signs of early lung cancer and pulmonary non-neoplastic disease. The main indexes of early lung cancer group were new Daxi point (left) tenderness, lung shu point (left) tenderness and new Daxi point (right) tenderness and the secondary indexes were lung (left ear) tubercle and lung (right ear).
