Objective To survey the change of islet B-cell function in patients with gout, and to explore the relationship between islet B-cell function and uric acid in patients with gout.Methods76patients with gout were recruited according to the diagnostic criteria, and at the same time,30healthy controls were enrolled. The patients with gout were divided into two subgroups:37gout with prediabic state and39gout with normal glucose tolerance. Serum extracted from fasting blood samples were analyzed for serum glucose, uric acid(UA), total cholesterol(TC), triglyceride(TG), creatinine, urea nitrogen, and insulin level. Insulin and glucose after2minute,4minute,6minute stimulation with L-arginine and30minute,60minute,120minute stimulation with Oral Glucose Tolerance Test (OGTT) were measured.Results The body mass index (BMI), waist circumference, fasting glucose(FPG), two hour glucose, UA, creatinine, urea nitrogen, TC, TG, fasting serum insulin(FINS), homeostasis model assessment insulin resistant index (HOMA-IR) in gouty group were higher than those in control group (t=2.245-12.032, P<0.05). The time of peak distributed differently between the two groups (as for L-arginine stimulation2min:100%vs27.6%(X2=10.337, P<0.05); OGTT30-60min:100%vs67.1%(X2=16.86, P<0.05)). The peak value of insulin stimulated by L-arginine and OGTT of gouty group were higher than those in the control group (t=2.538,5.521, P<0.05). The incremental plasma insulin area under the curves (AUCs) after stimulation of L-arginine and OGTT were both higher at gouty group than at the control group (t=2.806,6.179, P<0.0). The concentration of plasma insulin rose up to (7.5±3.8) times right after the stimulation of L-arginine in gouty group which was significantly lower than that of the control group (t=4.523, P<0.01). As for the subgroups, the time of peak delayed significantly after the stimulation of OGTT in the gout with prediabetics subgroup (X2=8.860, P<0.05). Multiple step regression analysis showed that regression model of islet secretion as a dependent variable was positively correlated with BMI, waist circumference and UA (t=2.646-4.479, P<0.05).Conclusion Islet β-cell function increases compensatively in patients with gout, the multiples of the peak level and the base level of the insulin secretion in the first phase islet secretion decline, the time of peak delays significantly in the first and second phase islet secretion. What’s more, the time of peak in the second phase islet secretion in gout with prediabetic state is even later than the gout with normal glucose tolerance. Uric acid is associated with islet β-cell dysfunction.
