Backgroud&ObjectiveWith the economic development, living standards gradually improved, great changes have taken place in lifestyle and diet structure, our country of type2diabetes prevalence rate increased year by year, has overtaken India to become the first country to global diabetes. The harm of chronic complications of diabetes is mainly, eventually lend to disability or even death, how to help patients with diabetes in delaying chronic complications, so good glycemic control is the primary task. Good blood sugar control needs to be correct and appropriate treatment. But the current treatment choice is not standardized, especially insulin use opportunity has not formed the unified standard. Though the detection of diabetic patients with C-peptide levels, according to the blood glucose and C-peptide level to explore whether patients need to use insulin treatment and patients currently using insulin can deactivate the insulin, and evaluation of curative effect of adjustment programmes, and further analysis of C-peptide level discontinuation of insulin therapy, provide the basis for the patients with insulin application of type2diabetes, insulin application more standardized. MethodsPatients with type2diabetes in the hospital were collected, standard steamed bread meal test and synchronous C-peptide release test, and detection of glycosylated hemoglobin(HbAlc), body mass index(BMI), test blood glucose and C-peptide level in each time point, and inquiring about the general data of patients with age, duration of disease, treatment etc. According to the patient’s current blood glucose and C-peptide level, adjust hypoglycemic scheme, the final selection of122patients met the study criteria, divided into two groups, insulin treatment group (oral hypoglycemic drug used in the past and now begin to use or with insulin treatment, group A) and stop using insulin treatment group (in the past to use insulin treatment and now use of oral hypoglycemic drug, group B).Comparison between the two groups of blood glucose, C-peptide and other indicators of diversity, and the change of the two groups before and after treatment of FBG,2hBG, and Pearson correlation analysis, the analysis of the indicators and C-peptide of the area under curve correlation, and multiple linear regression analysis of the influencing factors of C-peptide area under curve(C-PAUC).Results1. The comparison between two groups of indicators:the insulin treatment group age, duration of disease, each time C-peptide, each time CP/BG ratio, C-PAUC were lower than the stop using insulin treatment group; HbAlc, each time BG, glucose area under curve (GAUC) were higher, the difference was statistically significant (P<0.05).2. The Pearson correlation analysis showed that C-PAUC was positively correlated with age, duration, C-peptide level at each time and each time CP/BG ratio; and negatively correlated with each time BG, HbAlc and GAUC.3. Multiple linear regression analysis showed that variable entered the regression equation has GAUC,2hCP/2hBG,3hCP/3hBG. 4. Two group of patients with treatment after adjustment FBG,2hBG were decreased significantly,the difference was statistically significant(P<0.05).5. Descriptive statistics showed that stop using insulin treatment group of C-peptide secretion peak levels greater than3.5ng/ml accounted for83.76%, less than3.5ng/ml accounted for16.24%.Conclusions1. In type2diabetic patients with C-peptide level is closely related with age, duration of disease, blood glucose level.2. Type2diabetes mellitus patients treated with insulin, C-peptide secretion when peak level recovery to3.5ng/ml above, is likely to discontinue insulin to oral hypoglycemic drug treatment.
