Objective:To explore the methods and clinical results of the arthroscopic acromioplasty for the treatment of impingement syndrome of shoulder.Methods:23cases with impingement syndrome of shoulder were studied. There were19males and4females. The average age of patients was48years ranging from43to55years.16right shoulders and7left ones were involved. The average age of patients was48years ranging from43to55years. A11the patients had a chronic injury and painful history around shoulder. The Neer impingement test was presented in18eases, Hawkins test in20cases, positive arch pain test in17cases and Jobe test in13cases. All the patients had been received MR examinations. The AP and the supraspinatus outlet projection of the X-rays were obtained before surgery. The procedure of arthroscopic acromioplasty consisted of following steps:the first was arthroscopic inspection of the glenohumeral joint with posterior portal; the second was arthroscopic subacromial inspection and subacromial bursectomy with full radius resector; the third was release or resection of the coracoacromial ligament with plasma knife; the fourth was subacromial decompression with abrasive drilling; and the last was debridement of the soft tissue around the acromio-clavicular joint and resection of the the spurs. All the patient received early postoperative exercises. Results:All cases were available for follow up. The average time was8months ranging from4to12months. According to the UCLA scoring system, the average score was17.6+4.5and31.5±1.6pre-and post-operation respectively; the difference was of statistical significance(P<0.05). The results were16excellent,4good,3fair and0poor.Conclusion:Impingement is the common cause for the pain and dysfunction of the shoulder.The arthroscopic acromioplasty is a reliable solution. This surgery has many advantages, such as mini-invasion, rapid recovery and early rehabilitation.
