Purpose:Successful root canal treatment is dependent on detailed knowledge of the morphology of the root canal system. The aim of this study was to investigate the morphology of the root canal systems of premolars and the location of root canal orifices and apical foramina in a Chinese subpopulation using cone-beam computed tomography (CBCT). oMethods:A total of1492CBCT images of premolars were obtained from518patients. Tooth position, root canal morphology, the distance between the apical foramen and the anatomical apex and the distance between the two root canal orifices of premolars with two root canals were investigated.Results:Amongst the440mandibular first premolars,1.14%(n=5) of the teeth had a C-shaped root canal system,77.14%(n=335) had one root canal and22.05%(n=97) had two root canals, three root canals were observed in0.68%(n=3). Most of mandibular second premolars had one root canal(96.98%),2.27%of the teeth had two root canals, only one this tooth had three root canals.76%of the maxillary first premolars had two root canals, only20%of the teeth appeared one root canal. The root canal morphology of premolars showed great variance. The main types were type Ⅰ~type Ⅴ、type Ⅷ and C-shaped. The distance between the apical foramen and anatomical apex of premolars were approximately0-3mm. The distance between the root canal orifice and the anatomical apex of premolars with two root canals varied. The distance between the two root canal orifices of premolars with two root canals was approximately1-3mm.Conclusions:The root canal morphology of premolars showed great variance. CBCT scanning is an effective method for studying the root canal configuration. This study may help clinicians look for and locate the root canal orifice in root canal treatment.
