ObjectiveTo investigate the diagnostic value of diffusion-weighted imaging (DWI) and diffusion tensor imaging (DTI) for obstructive hydrocephalus and explore the value of DWI and DTI on the diffusion movement of water molecules in periventricular white matter in patients with hydrocephalus. Diffusion movement of water in periventricular white mater, the genus and the splenium of the corpus callousm was comparied between the patients with hydrocephalus and volunteers.Materials and MethodsThirty healthy volunteers as a control group and33patients with obstructive hydrocephalus, which were resulted from the tumor of the third or forth ventricle and comfirmed by operation, were included in this study. All cases were examined by GE company Signa HDx.3.0T superconductive MRI scanners. Conventional sequences (T1WI, T2WI and FLAIR), DWI and DTI were performed. All scan data was processed in the ADW. Worlstation4.0. DWI and DTI date was processing in Functiontool software. The statistical analysis was performed by SPSS13.0. All cases in2groups also underwent DWI and DTI examination and ADC maps, MD maps, FA maps, VR maps and RA maps were obtained from Functiontool software. All the parameters were measured on these maps and treated with independent sample t-test between the groups.ResultsThe ADC. MD, FA, RA, VR and AI of the bilateral periventricular white matter, the genu and the splenium of the corpus callosum were measured from in all cases. All data was normal distribution and treated with independent sample t-test. There was a statistical significance (p<0.05) of ADC values in the genu of the corpus callosum and RA values in the splenium of the corpus callosum between2groups. There was no statistical significance (p<0.05) of DTI parameters in the genu of the corpus callosum between2groups. There was a statistical significance (p<0.05) of MD, FA, VR and AI values in the bilateral periventricular white matter between2groups.Conclusions1、In this study, DWI and DTI technology can evaluate the diffusion of the water molecules and the anisotropy in the genu of the corpus callosum, the splenium of the corpus callosum and the bilateral periventricular white matter. The results exhibited that ADC values in the gune of the corpus callosum, the RA values in the splenium of the corpus callosum and MD, FA, VR, AI values in the bilateral periventricular white matter were statistical significance.2、Therefore, the DWI and DTI can be diagnosed hydrocephalus and may help tomake differential diagnosis of hydrocephalus.3、Combined with conventional MRI, DWI and DTI MR Perfusion technique can assessed Hydrocephalus comprehensively and accurately. The results provide a strong basis for clinical treatment.
