Objective This prospective cohort study took colorectal carcinoma patients of stage Ⅲ-Ⅳ as subject investigated.survival and quality of Life (QOL) as leading indicators, body weight change, the physical status score and tumor size as subordinate indicators.in order to improve the one-year survival rate of colorectal carcinoma patients by15%,with the method of traditional chinese medcine intervention in different tharepy stages of colorectal carcinoma,Eventually estabished propagable standard of clinical treatment.Methods The colorectal carcinoma patients of stageⅢ-Ⅳwho were in accordance with the inclusion criteria from5clinical units were divided into two groups-The group of integrated traditional chinese and western medicine and western medcine group according to different exposure factors.After baseline investigation.both groups adopt the same western medcine treatment(according to colorectal cancer NCCN clinical practice guidelines).while integrated traditional chinese and western medicine group was combined with traditional chinese medcine(TCM) tharepy in different western medcine tharepy stages(after surgery before chemotharepy.during chemotharepy. follow-up phase).During regular follow-up.the observation indicators such as treatment response,survival situation and recurrence and metastasis were recorded.Results(1) Survival situation:Six-months survival rate showed no significant difference between two groups.In one-year survival rate, integrated traditional chinese and western medicine group was8.47%higher than western medcine group(p=0.02),19.38%higher in two-year survival rate(p=0.000), both with significant difference。(2) Correlation of exposure factors and survival rate:Spleen deficiency, shortness of breath, dry mouth and throat are negative factors which have prognostic impact.The tharepy of spleen deficiency was beneicial to the increase of survival rate.(3) Change of symptoms of TCM:In the symptoms of TCM such as anorexia, lassitude, shortness of breath.tenesmic anus, dizziness and tinnitus,the integrated traditional chinese and western medicine group relieved more than western medcine group(p<0.05).(4) Karnofsky score:In the first diagnosis.the Karnofsky score of western medcine group was higher than integrated traditional chinese and western medicine group(p<0.05).while the six visit showed no significance between two groups(p>0.05). Chinese traditional treatment was beneficial to the stabilization of patients’physical condition.(5) Tumor size:According to Response Evaluation Criteria in Solid Tumors(RECIST), in the first diagnosis.the average tumor size of integrated traditional chinese and western medicine group was bigger than western medicine group(p<0.05), while the six visit showed no significance between two groups(p>0.05).which indicated that the tumor size of integrated traditional chinese and western medicine group was significantly reduced(p< 0.05).Conclusion(1) Based on the principles of strengthening the spleen and replenishing qi and the addition and subtraction with the change of patterns of syndrome,the therapeutic regimen of TCM should be established.(2) The research result of5clinical units showed that one-year survival rate of Ⅲ-Ⅳ colorectal carcinoma patients was raised by8.47%,and two-year survival rate19.38%.(3) The combination of TCM was able to reduce tumor size,Improve physical condition,relieve some TCM syndroms, enhance the patients’quality of life.
