OBJECTIVE:To improve young clinicians’knowledge of spontaneous perforation of the bile duct in infancy and childhood and clear diagnosis and treatment of it. METHODS:Report a case of children’s spontaneous perforation of the bile duct including its clinical manifestations, laboratory, diagnosis and treatment,and review the literature about it. DESIGN:Case report. PATIENT(S):A2-year-old female patient, complaining of abdominal pain, fever, nausea, vomiting, abdominal distension, abdominal tension, gurgling sound fading. WBC increasing, liver function damage, and confirmed biliary perforation by intraoperative. INTERVENTION:After the patient was admitted to our hospital, she accepted surgical treatment of Cholecystectomy, Choledochal cyst excision, hepaticojejunostomy of Roux-Y in general anesthesia. After the operation, the patient accepted anti-infective therapy, hemostasis. acid reducing, nutritional treatment. RESULTS:The patient was discharged from hospital after recovery and had no special symptom in three months later. CONCLUSION:spontaneous perforation of the bile duct in children is a rare disease. However, it has been described as the second most surgical cause of jaundice in infancy after biliary atresia. Incidence of the disease is relatively low. and it is lack of specificity of clinical manifestations, and is easily missed, misdiagnosed,caused by late treatment, nutritional status is poor, symptoms of systemic poisoning. Therefore, early diagnosis. positive surgical treatment is the key to reducing mortality.
