Objectives:To analysis the patients with acute brain stroke retrospectively, explore the clinical characters of pyrexia the related risk factors and prognosis.Methods:We screened the in-hospital patients within two years from January1,2010to December1,2011in Department of Neurology of Shandong Provincial Hospital, who were diagnosed with acute brain stroke, got the detailed clinical data of patients, according to case information, we try to analysis the relationship between the pyrexia and the other clinical factors in patients with acute brain stroke.Results:From2010to2011, a total of171cases of in-hospital patients with acute stroke, within which a total of69(40.4%) cases had ischemic stroke (59.1%) cases had hemorrhagic stroke and one case is mixed stroke.70(40.9%) cases have pyrexia and101(59.1%)cases are not pyrexia. In the pyrexia patients,42(60%) patients are due to infectious complications.The origin of infection most is respiratory tract (31,73.8%),5(11.9%) cases got urinary tract infection.11cases were died and they all had pyrexia. (1) Compared with the patients had not hyperthermia, the patients with pyrexia are most had high age (>65), stroke history, myocardial infarction history,dysphagia.invasive operation, hemorrhagic stroke,conscious disturbance and cerebral hemorrhage breaking into ventricle. There are no statistical significance about gender, hypertension history, diabetes history, malnutrition and smoking history, according to the risk factor analysis, the independent risk factor are high age (>65), stroke history,dysphagia,invasive operation,hemorrhagic stroke,conscious disturbance and cerebral hemorrhage breaking into ventricle.(2) Compared the patients had fever with infection and fever with not infection,the difference are high age (>65), dysphagia, hemorrhagic stroke and invasive operation,but the stroke history,myocardial infarction history,conscious disturbance and cerebral hemorrhage breaking into ventricle.(3) The duration of fist hyperthermia is longer in patients with infection than not(4)The mortality is higher in patients with pyrexia compared with the normothermia and hypothermiaConclusion:(1) Fever is a very common character after stroke and mostly caused by infection,especially the respiratory tract.(2) The relative clinical risk factor are high age (>65), stroke history,dysphagia,invasive operation,hemorrhagic stroke,conscious disturbance and cerebral hemorrhage breaking into ventricle.(3) The patients who had high age (>65), dysphagia, hemorrhagic stroke and invasive operation tend to got infectious fever which had longer duration of first hyperthermia.(4) According to this study, patients who had pyrexia have higher mortality than not.
