Objective: To explore the imaging manifestation of Magnetic resonanceangiography diagnose the complex Budd-Chiari syndrome and the value ofthree-dimensional reconstruction guiding for the intervention.Materials and Methods:42cases diagnosed by Color DopplerUltrasoud, and then those patients were examined by MRI and digitalsubtraction angiography. We use the1.5T GE SIGNA Torso front circle8channels. The Scan sequences are as follows:the Fiesta sequence,T1WI,T2WI and LAVA sequence. We get the MRI data transmission to GEAdvantage Wokstation4.3to reconstruct the three-dimensional image. Threepatients LAVA enhanced scanning sequence image were imported to theMimics software to reconstruct the three-dimensional image. To observe theinferior vena cava and hepatic vein vascular morphology, diameter andintra-cavitary signals, to analyse the inferior vena cava occlusion segment andcollateral circulation and to compare with angiography results. At the last toanalyse the results by SPSS15.0statistical.Result:(1)Magnetic resonance angiography display the hepatic vein,inferior vena cava sensitivity was92.06%, specificity was82.35%, MRI forBCS of the diagnostic coincidence rate was90%, in addition, we foundinferior vena cava thrombus8cases and inferior vena cava tumor sampleexpansion2cases;(2)The Complexity BCS patients inferior right hepatic veindisplay rate is as high as78.6%,Average diameter of blood vessels is9.2mm±3mm, Can be clearly showed lower right hepatic vein open position and angle,the relationship with hepatic vein;(3)Magnetic resonanceangiography can show the collateral circulation of BCS,it were divided intothe intrahepatic type, outward appearance and inside and outside the liver type,According to the specific vessel drainage area subdivided into nine subtypes;(4) The lesions of inferior vena cava were reconstruct by GE AW4.3workstation, That can be divided into four categories such as circular, cone,horizontal and diagonal, specific including7cases of circular arc form, cone26cases,5cases were horizontal, oblique4cases;(5) The3D model withbright color reconstruct by Mimics software,it is well showed that spatialrelationship of the hepatic vein, portal vein and inferior vena cava.Conclusion:(1)Magnetic resonance angiography can fully displayed thevascular lesions scope of the BCS,it is to provide exact image anatomy basisfor the complicated BCS;(2) GE AW4.3post-processing workstationreconstruction images can intuitive to show the obstruction sectionmorphology, inferior vena cava thrombus, inferior vena cava tumor andcollateral circulation of the complicated BCS;(3) The3-D structure of thecomplicated BCS reconstructed by Mimics software, it can be show thespatial relation of the inferior vena cava, the hepatic vein system and portalvenous system. It is important guiding significance that interventionaltreatment in the process of opening the inferior vena cava or hepatic veinocclusion segment with the complicated BCS.
