Treat Ascending Aortic Aneurysm With Aortic Valve Insufficiency By Applying Bentall’s Procedure

Abstract/Summary:
Object: To review the experiences in surgical treatment of ascending aorticaneurysm with aortic valve insufficiency by Bentall’s procedure.Method:31patients with ascending aortic aneurysm with aortic valveinsufficiency underwent Bentall operation in our departentment from Janaury2005to December2012, and their clinical data was retrospectively analyzed.Among these patients,58were men (74.19%),8were women(25.81%), whowere aged from25to61(average age42.2) and with the course of disease beingbetween24hours to30years.26cases were diagnosed as ascending aorticaneurysm(83.87%),5cases type A aortic dissection(16.13%),28cases with severeaortic valve insufficiency(90.32%),1case with severe aortic valve stenosis andmild aortic valve insufficiency(3.23%),2cases with moderate aortic valveinsufficiency(6.45%),6cases with Marfan syndrome(19.35%),5cases withmoderate or severe mitral valve insufficiency(16.13%),3cases with moderate orsevere tricuspid valve insufficiency(9.68%),5cases with pulmonaryhypertension(16.13%),pulmonary artery pressure41to106mmHg(average72.24mmHg), a case with patent foramen ovale(3.23%),2cases with atrialfibrillation(6.45%),a case with infective endocarditis(3.23%),2cases withhypertension(6.45%),2cases with tuberculosis(6.45%). One case’s cardiacfunction was level IV(3.23%),4cases’ was level III(12.9%),and26cases’ was level II(83.87%).one patient received emergency surgery(3.23%),and othersreceived elective surgery(96.77%).Results: In operations, cardiopulmonary bypass time was86to575min,blocking time was68to240min, average time of hospitalization18.1d. Twopatients died during operation, one patient received aortic arch graft replacement,2patients received bipolar radiofrequency ablation during the operation,2patientsrecived mitral valve annuloplasty,2patients received tricuspid valve annuloplasty,one patient received a second thoracotomy to stop bleeding because the bloodpressure continued to decline after the first operation. Other patients’ surgicalresults were good, with no postoperative complications observed. Follow-up:onepatient died of cancer. Others recovered well without any long-term complicationsand their cardiac function recovered to level I to level II.Conclusion: Bentall’s procedure is of optimal result and high success rate intreating ascending aortic aneurysm with aortic valve insufficiency, and thus is thefirst choice to treat ascending aortic aneurysm with aortic valve insufficiency.
