Hybrid Surgery To Improve Lower Limb Artery Segmental Block The Impotrance Of Ischemia

Objective: to study the hybrid s to improve lower limb artery occlusionsurgery clinical curative effect of ischemic lesions.Methods: a retrospective analysis in this paper in June2010to June2012to DSA/MRA/CTA and vascular color ultrasound examination were occlusionischemia in patients with lower limb artery segments in parallel hybrid surgeryin22cases. Operation method using iliac artery Fogarty bolt extraction for+iliac arterial lumen keratoplasty (PTA) and STENT (STENT) in6cases, iliacartery PTA++-femoral artery STENT artificial vascular bypass,4cases ofiliac artery PTA and STENT+stock total artery intima stripped resection(TEA)/deep arterioplasty8cases, iliac artery PTA++-popliteal artery STENTartificial vascular bypass grafting in4cases.22cases of patients withpostoperative follow-up, respectively through the above hybrid operation of thelong-term patency rate, complications, perioperative mortality, ABI, andoxygen partial pressure in the preoperative and postoperative change wereanalyzed.Results:22cases of patients with hybrid are successful after the surgery,postoperative limb skin temperature and blood flow, intermittent claudicationdistance gradually extended, resting pain feeling gradually reduce or disappear, from an average of0.27+/-0.13preoperatively ABI to an average of0.78+/-0.19postoperatively, oxygen partial pressure after surgery, up from10~30MMHG preoperatively in an average of35~48MMHG. No perioperativemortality and severe complications occurred. Group of22patients withpostoperative3months,6months,12months,18months and24months backto the hospital to review, clinical symptoms, lower limb artery colour to exceeddegree of ABI and ulcer healing.1patients during follow-up, there was1casedied.1case closed again amputation,1cases of distal of autologous artery stentin the narrow, line again after interventional treatment better. The rest areunobstructed.Conclusion: the multi-stage lower extremity arterial occlusive diseaseshould be adopted according to the condition and complications of personalizedtreatment plan. Hybrid operation for improving segment, the plane complex ofdiseased limb ischemia lesions has better clinical curative effect in the shortterm. At the same time with a small surgical trauma, faster recovery time,perioperative complications and lower mortality.
