Background: As the standard of living and lifestyle change, breast lesionshave been gradually become the main diseases endangering women’s health, breastmagnetic resonance imaging (MRI) has become one of the important methods ofbreast imaging examination, compared with mammography, ultrasound and otherexaminations, it has irreplaceable status among them.At present, the anatomicalmorphology research cannot meet the needs of mammary gland disease imagingdiagnosis, so functional imaging which could show us the microstructure of thephysiological or pathological manifestations, so diffusion weighted imaging of breastMRI play an important role on mammary gland disease diagnosis.Objective: To analysis the apparent diffusion coefficient (ADC)values of breastbenign and malignant lesions in MRI diffusion weighted imaging the and discuss thebest b value, in order to explore the clinical application in the differential diagnosis inbreast benign and malignant tumor, and to provide reliable evidence for clinical work.Materials and methods: Take the retrospective study, this study collected31patients with breast lesions and10volunteers as the research object, perform3.0TMRI scan diffusion weighted sequence, all results are confirmed by clinical andpostoperative pathologic result.and then transmit the original data to GE workstationwith Functcol software, adjust the threshold processing, and then determine the valueof b, the software automatically generate DWI value and the corresponding ADC canfigure, with reference to dynamic enhanced scan and image on the location of thelesions, mark interested in circular region, resurvey lesions and ADC values of normalbreast, the measured three times, average using the SPSS17.0statistical software forstatistical analysisResults:1. When the b value take600s/mm~2、800s/mm~2、1000s/mm~2, theADC value of normal breast tissue, benign lesions and malignant lesions arestatistically different, two further comparison, and ADC values change trend is malignant lesions <benign lesions <normal mammary gland.2. ADC values ofdifferent b values measured by the single factor analysis of variance is statisticallysignificant, after the two comparative analysis of normal breast tissue and benigngroup in ADC values under different b values did not see clear statistical significantdifference, while malignant lesions organizations exist with the increase of b value,the change trend of ADC values decrease, this trend has statistical analysis hassignificant meaning.3. When the b value takes800s/mm~2, the application of ADC toidentify benign and malignant tumors of the sensitivity and negative predictive value,compared with the rest of the two kinds of b value; When b value takes1000s/mm~2,measured the ADC value used to identify the specificity of breast benign andmalignant tumor, but in terms of accuracy, when b=800s/mm~2and b=1000s/mm~2,result is no clear difference.4. when b value takes600s/mm~2, area of ROC curve is0.549; when b value takes800s/mm~2, the area of ROC curve is0.636; when b valuewas1000s/mm~2,the area of ROC curve is0.626, we can conclude that b valuestake600,800and800was/s, ADC values used to identify benign and malignantbreast lesions was statistically significant, b value was800s/mm~2has a better than therest of the two values.Conclusion:(1) ADC values are quite different among normal breast tissue,benign and malignant lesions, and ADC values change trend is malignant lesions<benign lesions<normal mammary gland;(2) the normal breast tissue and benigngroup in ADC values under different b values did not see clear statistical significantdifference, while malignant lesions organizations exist with the increase of b value,the change trend of ADC values decrease;(3) when the b value takes800s/mm~2theapplication of ADC to identify benign and malignant tumors of the sensitivity andnegative predictive value,; When b value takes1000s/mm~2, the ADC used to identifythe specificity of breast benign and malignant tumor is better, but in terms of accuracy,when b=800s/mm~2s/mm~2and b=1000s/mm~2result is no clear difference;(4) theability to identify the benign and malignant breast lesions, b value was800s/mm~2hasa better than the rest of the two values.
