Objective:To explore the mechanism of prevention and protection of acupupuncture and moxibustion preconditioning on MIRI through observation of the effects of it on the ultrastructure of myocardium,CK,ET,PKC and HSP70of myocardium with ischemia-reperfusion injury in rabbits at different time.Methods:96New Zealand white rabbits were randomly divided into4groups: Sham operation group,Model group,Electroacupuncture preconditioning group and Moxibustion preconditioning group,24rabbits per group. Each group was divided into3small groups,8rabbits each small group.The3groups of Sham operation respectively underwent sham operation at Oh,24h,48h after pretreatment.The rest groups respectively underwent being modeled at Oh,24h,48h after pretreatment.The model of rabbits with MIRI was established by ligation of coronary artery and was made use after Ischemia for40minutes and reperfusion for60minutes.Using electron microscopy to observe the ultrastructure of myocardium in the ischemic region of left ventricular in each group, the contents of CK and ET in blood were detected with ELISA method and the contents of PKC and HSP70in myocardium were observed with immunohistochemical method. Results:l.The ultrastructure of myocardium in model groups had severe lesions,the structure was fuzzy,the alignment was indiscriminate,the muscle fibers were atrophic,the myofilaments were dissolved and necrotic, the interstitial substance was dropsical and the mitochondrias were dropsical and vacuolizationly.Compared with the model groups,the ultrastructure of myocardium in electroacupuncture and moxibustion preconditioning groups had milder lesions,the myocardial fibers were arranged more orderly,the range of necrosis and dissolution was limited and the structure of mitochondria was more normal.2.Compared with the subset groups of sham operation group, contents of CK in model groups was elevatory remarkably (P<0.05或P<0.01); Compared with the24h and48h subset groups of model groups, contents of CK in electroacupuncture preconditioning groups was obviously depressed (P<0.05)3.Compared with the subset groups of sham operation group, contents of ET in model groups was elevatory remarkably (P<0.01); Compared with the model groups, contents of CK in the24h and48h subset groups of electroacupuncture preconditioning groups and the48h subset groups of moxibustion preconditioning groups were obviously depressed (P<0.05)4.Compared with the model groups,contents of PKC in electroacupuncture and moxibustion preconditioning groups was increased at different degrees,but the most significant effects was emerged at48h after electroacupuncture preconditioning and24-48h after moxibustion preconditioning (p<0.05or p<0.01);Contents of PKC in moxibustion group was obviously higher than that in electroacupuncture group at24h after preconditioning (p<0.01)5.Compared with the model groups,contents of HSP70in electroacupuncture preconditioning groups was increased,but the most significant effects was emerged at48h after electroacupuncture preconditioning (P<0.01);Compared with the model groups,content of HSP70in moxibustion preconditioning groups was increased remarkably,but the most significant effects was emerged at24-48h after moxibustion preconditioning (p<0.01);Contents of HSP70in moxibustion group was obviously higher than that in electroacupuncture group at24h after preconditioning (p<0.01)Conclusion:1.The ultrastructure of myocardium on MIRI improved at different degrees in48hours after acupuncture or moxibustion pretreatment,which concludes that acupuncture and moxibustion preconditioning has preventive and protective effects on MIRI.2.Acupuncture and moxibustion pretreatment has protective effects on MIRI.Its mechanism may be related to decreasing the contents of CK and ET in blood and the increasing the contents of PKC and HSP70in myocardial,the time of best preventive and protective effects maybe at48h after preconditioning.3.The effect of electro-acupuncture pretreatment which reduced contents of CK in rabbit with MIRI may be better than that of moxibustion preconditioning;The effect of moxibustion pretreatment which increased contents of PKC and HSP70in rabbit with MIRI may be better than that of electro-acupuncture preconditioning.
