Changes And Significances Of Serum Ischemia Modified Albumin In Percutaneous Coronary Intervention

Objective To observe the changes and clinical significances of percutaneous coronary intervention (PCI) on transient myocardial ischemia by observing the levels of ischemia modified albumin(IMA), cardiac triponin T (cTnT) and creatine kinase MB (CK-MB). Methods123patients with coronary heart disease received PCI,100patients with coronary heart disease only received coronary angiography (CAG), not PCI,and93patients received CAG,but were confirmed normal coronary artery by CAG. All the patients above, blood samples were collected for determining IMA, cTnT and CK-MB3hours before and after the operation.The clinical data of all the three groups were record:age, gender, fat, smoking, hypercholesteremia, hypertension, diabetes, left heart function, CAD family history, old myocardial infarction, treated vessels, and so on. The30patients of the PCI, whom main adverse cardiac events took place to, blood samples were collected for determining IMA, cTnT and CK-MB3hours and10hours before and after the operation. We also recorded the relationships between the IMA and the max inflation pressure, the duration of total inflations and the number of inflations. Results IMA levels were elevated from the baseline to3hours after PCI in123patients (p<0.05). However, IMA levels were not significantly different3hours after CAG in193patients (p>0.05). Levels of cTnT and CK-MB remained unchanged3hours after the operation in all patients of the three groups (p>0.05). Levels of IMA,cTnT and CK-MB were all at normal reference value. IMA levels of the PCI group were elevated compared with that of the two CAG groups, respectively (p<0.05). Levels of cTnT and CK-MB remained unchanged of the three groups (p>0.05). In the30patients of the MACE the sensitivity of IMA was higher than that of cTnT and CK-MB (p<0.05).Levels of IMA were significantly related with the max inflation pressure (R=0.8985,p=0.002), the duration of total inflations (R=0.861,p=0.000) and the number of inflations (R=0.976, p=0.000). Levels of IMA were not related with the clinical data:age、gender, fat, smoking, hypercholesteremia, hypertension, diabetes, left heart function, CAD family history, old myocardial infarction, treated vessels, and so on (p>0.05).Conclusion Because PCI can cause light injuries on myocardium, levels of IMA were quickly increased and correlated with severity of myocardial ischemia and before significant elevations of cTnT. Ischemia modified albumin is not only a sensitive marker of myocardial ischemia, but also earlier than both cardiac triponin T and creatine kinase MB.
