Assessment Of The Changes Of Right Ventricular Function Before And After Transcatheter Closure In Patients With Atrial Septal Defect By Single Beat Real Time Three Dimensional Echocardiography

Objectives The study aimed to investigate the changes of right ventricular systolic function before and after Transcatheter closure of atrial septal defect(ASD) by single beat real time three dimensional echocardiography (sRT3DE).Participants and Methods The study compared the results of the two groups of participants, namely the experimental group and the control group. In the experimental group, there were24participants with Ostium Secundum Defect, among which5were males and19females, with the average ages29.62±19.23years.The control group consisted of20healthy subjects,9males and11females, with the average ages24.45±17.70years. Right ventricular end-diastolic volume(RVEDV)、right ventricular end-systolic volume(RVESV)、right ventricular stroke volume (RVSV) and right ventricular eject fraction (RVEF) of the two groups of subjects were measured before the Transcatheter closure, ASD group were measure at2、15after30days after transcatheter closure respectively by sRT3DE. After that, the author calculated right cardiac index (RVCI) with body surface area(BSA), and adopted ways of Independent samples T-test, variance analysis, and correlational research to compare the two groups.Results According to the results, the RVEDV, RVESV, RVSV, RVEF, and RVCI of the experimental group were significantly higher than those of the control group (p<0.05). All indexes decreased significantly (p<0.05)2days after the closure of ASD and the right ventricular volume and RVCI were higher than those of the control group (p<0.001) while the RVEF was lower than that of the control group (p<0.001).15days after the closure of ASD, all indexes showed no difference compared with data in the previous stage (p>0.05). Compared with those of the control group, the right ventricular volume and RVCI were higher (p<0.05), while RVEF continued to be lower (p<0.05).30days after the closure of ASD, RVEDV, RVESV, and RVCI decreased greatly (p<0.05) compared with those after15days’ of closure. RVSV, RVEF showed no significant difference(p>0.05). The right ventricular volume and RVCI were higher than those of the control group (p<0.05) and RVEF was normal. The maximal diameter of atrial septal defect showed a positive correlation with RVCI(r=0.623, p<0.01), and age had a negative correlation with RVCI (r=-0.495, p<0.05)Conclusion The single beat real time three dimensional echocardiography is a new technology which can assess right ventricular function quickly and accurately. The right ventricular volume and function of patients with ASD are higher than those of normal patients before transcatheter closure. Two days later after transcatheter closure, the right ventricular volume and function are lower than those before therapy.15days after the closure, all indexes showed no difference compared with data in the previous stage. And they began to recover30days after Transcatheter closure. The more diameter and the age of patients with ASD, the slower the body recovers.
