PKP Versus PVP For Treatment Of Osteoporotic Vertebral Fractures: A Systematic Review

BackgroundCurrently, percutaneous kyphoplasty(PKP) and percutaneous vertebroplasty (PVP) has been widely used in minimally invasive surgical treatment of osteoporotic vertebral fractures in clinical, which procedure should be selected there is no strict standards.PurposeTo make a systematic review from the randomized controlled trials and controlled clinical trials literature of PKP versus PVP for treatment of osteoporotic vertebral fractures published at home and abroad, which provide some evidence for clinical application.MethodThe Cochrane Library, PubMed, EMBASE, Science Citation Index database, the Biomedical Literature Database, CNKI, CQVIP, WANFANGDATA were searched by computer from January1998to January2012. The comprehensive search engines of Google, Yahoo and Baidu were searched by manual search. The systematic review could be done after analyzing the useful data and explaining the result.ResultA total of5related papers were included.There was no significant difference between PKP and PVP in VAS score after1month,6months and1year postoperatively(P>0.05). There was no significant difference between PKP and PVP in the degree of back pain and Oswestry scores(P>0.05). The postoperative vertebral height was no significant difference between the two groups (P>0.05), but sensitivity analysis showed that the credibility of the results. PKP was lower than PVP in vertebral height compression ratio after1month,6months and1year postoperatively (P<0.05). PKP was lower than PVP in kyphotic wedge angle after1month,6months and1year postoperatively (P<0.05). There was no significant difference between PKP and PVP in operative time, amount of bone cement and adjacent vertebral fractures(P>0.05). PKP is better than PVP in the safety of bone cement leakage.ConclusionPKP is better than PVP in the restoration of vertebral height and Correction of vertebral kyphosis. PKP is better than PVP in the safety of bone cement leakage.
