Objective:To analyze the clinical features, treatment and factors affecting the prognosis ofuterine sarcomas with different histological types, and to provide clinical datas for thediagnosis and treatment of uterine sarcoma.Metarials and medthods:89cases of uterine sarcoma patients treated at The Second Hospital of JilinUniversity between8/2007and8/2012were followed up to collect clinicalcharacteristics, treatment and status right now. SPSS20.0was used for statisticalanalysis, the enumeration datas of of the clinical features was compared by χ2test,and the measurement datas was compared by t test. The survival rate of the uterinesarcomas was compared by Kaplan-Meier, and the single factors were analyzed usinglogistic regression, and multi-factor analysis were analyzed using the Coxproportional hazards model analysis.P <0.05was the standard of statisticalsignificance in each analytical test method.Results:1. Clinical features1.1AgeThese89uterine sarcomas’ mean age was51.74±1.524years, median age was51years, difference was significant among the mean age of different histological types(P=0.005).1.2MenstruationAmong these89uterine sarcomas,48patients were pre-menopause and41patients were post-menopause, the difference between both groups wasstatistically significant (P=0.001).1.3Clinical manifestationThe most common clinical manifestation of89uterine sarcomas is vaginal bleeding, and vaginal bleeding is more common in CS than LMS, and the differencebetween both groups was statistically significant (P=0.009).1.4Tumor markersThere was no significant difference in the increase of CA125and CA199amongdifferent histological types (P=0.382,0.523).1.5Clinical stagingThere was no sifnificant difference in clinical staging among differenthistological types (P=0.125).1.6Expression of estrogen and progesterone receptorThere was no sifnificant difference in the expression of estrogen andprogesterone receptor among different histological types (P=0.560).1.7Metastasis of lymph nodesThere was no sifnificant difference in the metastasis of lymph nodes amongdifferent histological types (P=0.320).2. Survival rate and prognostic analysis of uterine sarcomas2.1Survival rateThe1,2,5-year survival rate of89uterine sarcomas were82.6%、73.7%、67.5%.2.2Univariate analysis of prognosisThe unfavorable factors which affect uterine sarcomas’ survival rate were age,clinical staging, myometrial invasion, pelvic lesions, the favorable factors whichaffect uterine sarcomas’ survival rate were treatment, expression of estrogen andprogesterone receptor.2.3Multiplicty of prognosisThe independent factors which affect uterine sarcomas’ survival rate wereclinical staging and treatment.Conclusion:1. The age and menstrual status are different between uterine sarcomas with differenthistological types, most of the patients with LMS and ESS are below51years andpre-menopause.2. The most common first clinical manifestation of uterine sarcomas is vaginal bleeding, and vaginal bleeding is more common in CS than LMS.3. Operation is the main treatment of uterine sarcomas, but enlarged operation couldnot raise the survival rate.4. Chemotherapy and radiotherapy after operation could raise the survival rate ofuterine sarcomas.5. The1,2,5-year survival rate of uterine sarcomas were82.6%、73.7%、67.5%.6. The unfavorable factors which affect uterine sarcomas’ survival rate were age,clinical staging, myometrial invasion, pelvic lesions, the favorable factors whichaffect uterine sarcomas’ survival rate were treatment, expression of estrogen andprogesterone receptor.7. The independent factors which affect uterine sarcomas’ survival rate were clinicalstaging and treatment.
