Objective:Research the position and distribution of accessory nasal cavity ostiumand circumjacent structures.Use the structures as the investigative object whichcan be used for a marker by mycteric endoscope. Measure the correlative dataabout the accessory nasal cavity ostium and marker so as to find the way to fixthe ostium and important marker. Providing the anatomical basis for lateralwall of the nasal cavity structure for clinical endoscopic findingMaterials and Methods:Select40anult cadaver heads(80cases) which have been fixed byformalin,with27male and13famale.There are not surgical trauma,ormationand operating ransform,all of them are without age. Anatomise the cadaverhead along sagittal diameter,remove nasal septum and reveal the investigativeregion. Research the relationship of structures at lateral wall of the nasal cavityand measure the correlative data.All the data have been analysed by SPSS11.5.Results:(1)The sagittal diameter of ostium maxillaries is(17.60±2.08)mm,themaximum value is23.24mm, the minimum value is14.68mm. The verticaldiameter of ostium maxillaries is (11.27±2.10)mm,the maximum value is16.08mm, the minimum value is7.42mm.The distance between anterior borderof ostium maxillaries and uncinate process is (3.83±1.25)mm, the maximumvalue is6.44mm, the minimum value is1.64mm. The distance betweenposterior border of ostium maxillaries and uncinate process is(12.74±2.23)mm,the maximum value is17.62mm, the minimum value is7.08mm.(2)The sagittal diameter of endostoma maxillaries is(6.95±0.34)mm,themaximum value is7.18mm, the minimum value is6.74mm.The verticaldiameter of endostoma maxillaries is (2.50±0.21)mm,the maximum value is 2.74mm, the minimum value is2.10mm. The distance between ostiummaxillaries and endostoma maxillaries is (7.62±0.64)mm, the maximum valueis7.80mm, the minimum value is7.44mm.(3)The distance between anterior superior extremity of uncinate processand superior attachment point of middle nasal concha is (0.62±0.16)mm,themaximum value is0.88mm, the minimum value is0.36mm. The verticaldistance between posterior inferior extremity of uncinate process andattachment point of inferior nasal concha is (1.32±0.41)mm,the maximumvalue is1.82mm, the minimum value is0.98mm. The distance between anteriorsuperior extremity of uncinate process and anterior border of aperture of frontalsinus is (0.52±0.10)mm,the maximum value is0.70mm, the minimum value is0.36mm. The distance between posterior inferior extremity of uncinate processand posterior border of aperture of frontal sinus is (1.07±0.26)mm,themaximum value is1.52mm, the minimum value is0.78mm.The length ofuncinate process is (16.88±3.47)mm, the maximum value is19.06mm, theminimum value is11.92mm. The height of uncinate process is (4.72±0.58)mm,the maximum value is5.14mm, the minimum value is3.28mm. The distancebetween middle point of nasoturbinal concha and superior attachment point ofmiddle nasal concha is (1.95±0.38)mm,the maximum value is2.52mm, theminimum value is1.60mm.(4)The sagittal diameter of ethmoidal bulla is (9.73±3.10)mm,themaximum value is12.18mm, the minimum value is5.74mm. The verticaldiameter of ethmoidal bulla is (12.50±4.72)mm,the maximum value is18.58mm, the minimum value is6.60mm. The distance between middle pointof ethmoidal bulla and nasal spine is (31.62±7.46)mm,the maximum value is40.98mm, the minimum value is22.50mm. The distance between middle pointof ethmoidal bulla and the attachment border of inferior nasal concha is (12.88±4.63)mm,the maximum value is18.32mm, the minimum value is7.60mm.(5)The distance between the most superficial point and nasal columella is(61.82±5.74)mm,the maximum value is67.20mm, the minimum value is55.72mm. The distance between the most superficial point and inferior borderof aperture of sphenoidal sinus is (61.82±5.74)mm,the maximum value is6.02mm, the minimum value is5.14mm. The distance between the mostsuperficial point and middle point of sphenopalatine foramen is(9.28±0.79)mm,the maximum value is10.06mm, the minimum value is8.30mm. The distance between the most superficial point and postnaris is(7.80±0.64)mm,the maximum value is8.62mm, the minimum value is7.10mm. The distance between the most superficial point and anterior nasalspine is (49.37±6.27)mm,the maximum value is57.24mm, the minimum valueis40.88mm.(6) The sagittal diameter of aperture of sphenoidal sinus is(5.18±0.43)mm,the maximum value is6.02mm, the minimum value is4.62mm. The vertical diameter of aperture of sphenoidal sinus is(3.16±0.33)mm,the maximum value is3.54mm, the minimum value is2.88mm. The distance between aperture of sphenoidal sinus and postnaris is(15.34±2.70)mm,the maximum value is18.92mm, the minimum value is11.76mm. The distance between aperture of sphenoidal sinus and the end ofsuperior nasal concha is (9.67±1.21)mm,the maximum value is12.04mm, theminimum value is8.36mm. The distance between aperture of sphenoidal sinusand the end of superior nasal concha is (9.67±1.21)mm,the maximum value is12.04mm, the minimum value is8.36mm. The distance between aperture ofsphenoidal sinus and the end of superior nasal meatus is (12.72±1.85)mm,themaximum value is15.58mm, the minimum value is10.72mm. The distance between aperture of sphenoidal sinus and the end of middle nasal concha is(20.04±2.36)mm,the maximum value is23.44mm, the minimum value is17.28mm. The distance between aperture of sphenoidal sinus and the end ofmiddle nasal meatus is (25.62±2.61)mm,the maximum value is29.62mm, theminimum value is22.76mm. The distance between aperture of sphenoidal sinusand posterior ethmoid sinus ostium is (6.08±1.53)mm,the maximum value is7.80mm, the minimum value is4.28mm.(7)The transverse diameter of aperture of frontal sinus is(3.57±0.83)mm,the maximum value is4.28mm, the minimum value is2.52mm.The vertical diameter of aperture of frontal sinus is(1.72±0.27)mm,the maximum value is1.96mm, the minimum value is1.30mm. The middle sagittal diameter of frontal sinus is (9.25±2.73)mm,themaximum value is12.54mm, the minimum value is7.02mm. The distancebetween superior inferior attachment point of middle nasal concha and apertureof frontal sinus is (10.26±2.61)mm,the maximum value is13.94mm, theminimum value is7.68mm.(8)The distance between head of uncinate process and aperture of frontalsinus is (7.85±1.53)mm,the maximum value is10.26mm, the minimum valueis5.88mm. The distance between the frontal wall of ethmoidal bulla andaperture of frontal sinus is (4.62±0.87)mm,the maximum value is5.94mm, theminimum value is3.52mm. The distance between nasoturbinal concha andaperture of frontal sinus is (9.27±1.02)mm,the maximum value is11.08mm,the minimum value is7.62mm.Conclusion:(1) Apertura maxillaries is in middle nasal meatus, inferior interior ofuncinate process. The distance between the ostium and the head and end ofuncinate process.are4mm and13mm. Go straight through apertura maxillaries about8mm,we can pass the narrow endostoma maxillaries,and go intomaxillary sinus.(2) Ucinate process,nasoturbinal concha and ethmoidal bulla are allsituated in middle antrum region. Ucinate process is at inferior part and parallelwith middle nasal concha. nasoturbinal concha and ethmoidal bulla are situatedat anterior superior and posterior superior part of ucinate process. Clearpositioning on the three elements is a great significance about opening theoperation field and removing the specific structures(3) There is a significance about fixing the anterior region of sphenoidalostium,which can be used to define and find the aperture of sphenoidal sinusand surrounding structures.This research provide an anatomic basis for findingaperture of sphenoidal sinus and discriminate between aperture of sphenoidalsinus and posterior ethmoid sinus ostium.(4) Aperture of frontal sinus is in a naeeow gap which make up by thehead of uncinate process, the anterior wall of ethmoidal bulla and nasoturbinalconcha.It can be used to fix the aperture of frontal sinus,which is the distancebetween the aperture and the three apexes.This method provides an anatomicbasis for fixing the ostium under the mycteric endoscope,and it have a guidancefor clinical operation.
