Study Of The Hospital Expenses And Loss Distribution Of New Rural Cooperative Medical System In Jilin Province

As one of the four New Rural Cooperative Medical System (NRCMS) trials,Jilin province started their work on NRCMS since2003Jul. Completely reached thegoal of NRCMS full coverage in Jilin province at2007, one year before the proposedtimeline. Till the end of2012, Jilin province went through10years of proceeding andadventures on NRCMS mechanisms. Got progress on revising the details of NRCMS,developed the NRCMS, and improved rural residents’ health levels. The NRCMSconfigured the problems of difficult on seeing doctors and unaffordable expenses forrural residents. Good for rural residents, developed the medical system and thegovernment won their support. As the NRCMS proceeding for ten years, there are stillproblems accumulating, increased expenses for either government or rural residents,since the medicine expenses increased. Now the hospitals in Jilin province which takeNRCMS trials are having problem on increased expenses, the patients and the fund ofNRCMS are leaving rural area for higher level hospitals. If the hospital cannot besolved as soon as possible, the rural residents would have to afford even moreexpenses. The benefits of rural residents from NRCMS would be stopped by theincreasing expenses; the NRCMS would finally be stopped by the increasing expensesas well. To study the hospital expenses and the loss distribution of NRCMS will beimportant for control medicine expenses, develop NRCMS in Jilin province.There are four parts in this thesis:First, we summarized the results of NRCMS of Jilin province over the pastdecade. Pointed that the cost of Jilin NRCMS was increasing, to control the hospitalexpenses is extremely urgent.Second, we reviewed the historical background and the policy of NRCMS,which lead to the unreasonable expenses. We should study how to control the hospital expenses under the whole society and history background, not only depend on forcingpolicy or lowering the prices, which wouldn’t work on this point.Third, we studied the international classic health insurance models, and thecharacters of its expenses control, and compared to our health insurance policy andexpenses control. We provided data for researchers to develop Chinese NRCMS andmedicine expenses control.Forth, we provided new methods to control the increasing expenses：to acceleratethe NRCMS legislation; to build city-rural integration policy for health care; to buildcompensate policy for medical organizations; to develop NRCMS payment revolution,etc.
