Endovascular Treatment Of Dural Arteriovenous Fistulas

ObjectiveDepending on the different location and different venous drainage pattern of DAVFs,patients may experience symptoms variable, ranging from mild symptom to fatalhemorrhage, and often need effective treatment. The purpose of the treatment is permanentocclusion of the fistulas. We use Onyx as the main endovascular treatment material to curedifferent kind of DAVFs, to investigate the clinical symptoms, the strategies of treatment,the technique and the efficacy depending on location and drainage pattern of DAVFs.MethodsA retrospective review of10patients with DAVFs from Tongji hospital of Tongjimedical college of Huazhong Science and Technology University was performed. In all10patients,8were male and2were female, their age range from34to61. Two patients wereless than forty years old, seven patients were between forty and sixty, one patient was oversixty, and with an average of forty-six. The clinical characteristics, therapeutic results andcomplications before and after therapy of the patients were analyzed.ResultsThe10patients had12DAVFs. One of them showed dizziness, two showed headacheassociated with intracranial murmur, two showed ocular symptoms, and five showedintracranial hemorrhage. Ten DAVFs of nine patients were occluded with single catheterizeand single injection. One patient had partial embolization. One patient with two DAVFs, weoccluded the bleeding responsibility DAVF, and left the asymptomatic anterior cranial fossaDAVF for further treatment. All patients had a good outcome without a death. TheFollow-up showed no recurrence and no neurological dysfunction. Endovascular treatmenthad achieved a satisfactory result.ConclusionDepending on the different location and different venous drainage pattern of DAVFs,patients may experience symptoms variable, and the therapies were complicatd. With theprogression of the materials and technology of the endovascular treatment, it can achievesatisfactory result in treating different kind of DAVFs.
