Objective:To initially identify the best Moxibustion times to treat Primary Dysmenorrhea (PD) with Moxibustion by comparing the therapeutic effects of treating PD in Guan Yuan acupoint with Moxibustion in different times (①from5days prior to menstruation to one day before menstruation,qd, a total of five times;②from5days prior to menstruation to the second day of menstruation,qd, a total of7times), and we expect to find out the functional mechanism in the field of reproduction&endocrine.Method:90cases of PD were divided into3groups at random,30cases in group A,30cases in group B and control group respectively. The two treatment groups were treated in Guan Yuan acupoint with Moxibustion in different times respectively (from5days prior to menstruation to one day before menstruation and to the second day of menstruation),qd. The blank control group was recorded the observation projects in various scales every menstrual cycle timely without treatment. All continued three menstrual cycles. After treatment, the total time of abdominal pain, the level of abdominal pain and the values of Retrospective Symptom Scale would be observed in each group. Meanwhile, the vein blood in the second day of menstrual flow in the menstrual periods before and after treatment will be collected, tested and compared PGE2、PGF2α and PGF2a/PGE2between the two Moxibustion groups.Result:1. Comparison with the result of before and after treatment within each moxibustion groups (group A and group B) in the level of abdominal pain(VAS), both of group A and group B had significant differences(P<0.05). Comparison with the difference of VAS score before and after treatment within the two moxibustion groups had no significant difference (P>0.05). After treatment, group A and group B were in contrast with the control group respectively in VAS score difference, there were statistically significant differences (P<0.01)2. Comparison with the result of before and after treatment within each moxibustion groups in total time of abdominal pain, both of group A and group B had significant differences(P<0.01). Comparison with the difference of the total time of abdominal pain before and after treatment within the two moxibustion groups had no significant difference (P>0.05). After treatment, group A and group B were in contrast with the control group respectively in the total time difference, there were statistically significant differences (P<0.05)3. Comparison with the result of before and after treatment within each moxibustion groups in the values of Retrospective Symptom Scale(RSS) had no significant difference (P>0.05). After treatment, group A and group B were in contrast with the control group respectively in difference of the values of RSS, there were not statistically significant differences (P>0.05)4. Comparison with the test result of PGF2a in vein blood before and after treatment within respective groups, group A and group B had significant differences (P<0.05) respectively, but there were not statistically significant differences (P>0.05) in differences of the result of PGF2a in vein blood before and after treatment within the two moxibustion groups.5. Comparison with the result of PGE2in vein blood before and after treatment within respective groups, group A and group B had no significant difference (P>0.05).And there were not statistically significant differences (P>0.05) in differences of the result of PGE2in vein blood before and after treatment within the two moxibustion groups.6. Comparison with the result of PGF2α/PGE2in vein blood before and after treatment within respective groups, group A and group B had significant differences (P<0.05) respectively, but there were not statistically significant differences (P>0.05) in differences of the result of PGF2α/PGE2in vein blood before and after treatment within the two moxibustion groups.7. The Clinic effects of group A and group B had no significant differences (P>0.05).Both of them had obvious effects for treating PD.Conclusion:1. Both of the two moxibustion groups for Primary Dysmenorrhea(PD) have very good improvement action and curative effect in abdominal pain (VAS) and the total time of abdominal pain without distinction, We can choose the times of from five days before menstruation to one day before menstruation for treating PD with Moxibustion in order to save medical resources as well as the effort and time of doctors and patients.2. There was no obvious curative effect on the values of Retrospective Symptom Scale(RSS).3. Both of the two treatment groups for Primary Dysmenorrhea(PD) can lower the level of PGF2α and PGF2a/PGE2in vein blood. It showed that Moxibustion treatment can decrease PGF2a and PGF2α/PGE2in order to eliminate spastic contraction of the muscle of uterus in the field of reproduction&endocrine, which may be the curative mechanism of treating PD with Moxibustion.4. The related pain factors of PD caused by PGs secretion reached a peak within48hours of menstruation. Moxibustion from5days prior to menstruation to the second day of menstruation comply with this peak, but the two treatment groups have no obvious distinction on the reduction of PGF2α content in the peripheral blood and the ratio of PGF2α/PGE2. In the first one to two days of the menstrual PGs in the secretion reached peak increasing moxibustion may have no fuction on further reduction. Maybe it need more accumulation of moxibustion treatment. All of these are need to be further studied.
