Effect Of Combined Use Of Diedabuguwan And Nimodipine On Glucocorticoid-induced Femoral Head Necrosis And Clinical Observation

The first part of the experimentObjective:To study the combined use of Diedabuguwan and nimodipine for the treatment and prevention of early steroid osteonecrosis of the femoral head.Method:Sixty adult New Zealand white rabbits were divided randomly into5groups:A combination group (n=12), A Chinese traditional medicine group (n=12), A nimodipine group (n=12), A model group (n=18) and the control group (n=6). The first four groups were set up animal models of steroid-induced avascular necrosis of femoral head by combining Methylprednisone sodium succinate with equine serum, and the control group weie injected partes aequales normal sodium, the combination group and the Chinese traditional medicine group were started to drench the solution of Diedafuguwan(3g/kg, dissolved with20ml normal sodium) from experiment begining. The combination group and the nimodipine group were began to inject nimodipine into ear-vein (0.1mg/kg) every day, keeped on injecting two weeks.The model group and control group were injected partes aequales normal sodium, and drenched20ml normal sodium. After the experiment at4,8weeks, serum cholesterol, triglyeride and plasma viscosity were tested.We observed the necrosis of sample through pathological section.Results:(1) Compare the combination group with the Chinese traditional medicine group and the nimodipine group, serum cholesterol, triglyceride and plasma viscosity were no significant difference(P>0.05);(2) Compare the Chinese traditional medicine group and the nimodipine group with model group, serum cholesterol, triglyceride and plasma viscosity were significant difference(P<0.05);(3) Compare the combination group, the Chinese traditional medicine group, the nimodipine group with model group, the pathological view of femoral head were obviously reduced.in first three groupsConclusion:The treatment of early steroid osteonecrosis of the femoral head by the combined use of Diedabuguwan and nimodipine, can significantly reduce serum cholesterol, triglyceride and plasma viscosity,and improve the pathology of the bone cells and capillaries of the femoral head.The second part of the clinical preliminary observationObjective:To observe the treatment of early steroid osteonecrosis of the femoral head by the combined use of Diedabuguwan and nimodipine.Methods:Twenty outpatient with early steroid osteonecrosis of the femoral head were treated with the combined use of Diedabuguwan and nimodipine. The course of the treatment was90days.The accumulated points of plan between prior treatment and post treatment, serum cholesterol, triglyceride and plasma viscosity of prior treatment and post treatment,curative effect of prior treatment and post treatment were studied and statistically analyzed.Results:(1) The total effective rate was95%(19/20);(2)Between prior and post treatment, the patients pain points and Harris hip score was improved significantly, serum cholesterol and triglycerides, whole blood viscosity was also reduced significantly, with significant difference (P<0.05);(3) After the treatment, X-ray suggests that the original avascular necrosis of the department to repair.Conclusion:The treatment of combined use of Diedabuguwan and nimodipine can effectively improve the clinical symptom of the avascular necrosis of the femoral head.
