Objective:To explore the features of TCM syndrome elements in Type2diabetes mellitus patients with diabetic nephropathy.Methods:260type2diabetes mellitus patients were investigated.Meanwhile,the indexe of TCM syndrome elements dierentiation was used in137patients with diabetic nephropathy,and123non-diabetic nephropathy as control, and sort out the characters ofsyndrome key factors. And then analyze the evolving rule of the syndrome key factors of DN patients among the DN Ⅲ, DN Ⅳ and DN V groups.Results:①The levels of age, duration of disease, urea nitrogen, creatinine, uric acid, hypertension and HDL-C were significantly different(P<0.01) between DN and Non-DN groups.②The main locations of disease in Type2diabetes mellitus patients with diabetic nephropathy were kidney (68.6%), liver (56.9%), and spleen (32.8%). In asthenia syndromes in Type2diabetes mellitus patients with diabetic nephropathy, the integrals of syndrome elements were gradually reduced from Yin deficiency (80.3%) to blood deficiency (62.0%) to Qi deficiency (51.1%) to Yang deficiency(51.1%). In sthenia syndromes in Type2diabetes mellitus patients with diabetic nephropathy, the integrals of syndrome elements were gradually reduced from Qi stagnation to dampness (69.3%) to phlegm (67.9%) to heat (64.2%) to blood stasis (31.4%).③Among the pattern factors of five viscera in Type2diabetes mellitus patients with diabetic nephropathy, kidney occupied first place(P<0.05); When it came to DN Ⅴ patients, spleen occupied first place (P<0.05).④The integral of syndrome elements of spleen, blood deficiency and Qi deficiency in DN V patients were significantly higher than that in Non-DN, DN Ⅲ and DN Ⅳ groups respectively (P<0.05).⑤BMI was positively related to dampness and phlegm of syndrome factors (P<0.01); TG was positively related blood deficiency and blood stasis of syndrome factors (P<0.05).⑥Urea nitrogen was positively related to spleen, lung, Qi deficiency, blood deficiency, Yang deficiency, cold, heart mind and water stasis of syndrome factors (P<0.011或P<0.05); Creatinine was positively related to spleen, heart, heart mind, Qi deficiency, blood deficiency, Yang deficiency, Jing deficiency, cold, and water stasis of syndrome factors (P<0.01或P<0.05)Conclusions:Location syndrome elements of DN were kidney, liver and spleen. Common nature syndrome elements of DN were Qi deficiency, blood deficiency, Yin deficiency, Yang deficiency, dampness, phlegm, heat and blood stasis. Spleen and water stasis were the evolving rule of the syndrome key factors in DN V group. With the development of DN, spleen, blood deficiency, Yin deficiency and dampness of syndrome elements were increasingly serious. There are characters and evolving rules of its own in focuses and syndrome key factors in Type2diabetes mellitus patients with diabetic nephropathy. There was a close relationship between BMI and dampness and phlegm of syndrome factors. Meanwhile there was also a close relationship between TG and blood deficiency and blood stasis of syndrome factors, There was difference between abnormal renal function and the kidney pathological features of TCM. There was significant difference in the duration of disease, blood glucose, blood pressure, blood lipids, blood urea nitrogen, creatinin e, uric acid of the two groups.
