ObjectiveThis study aimed to explore the lowered extent of HBV-DNA and HBsAg in the peripheral blood by DFPP. as well as the relationship between different amount of circulating plasma and the clearance of Hepatitis B Virus.MethodsCollected5500mL plasma which contains HBV and is replaced by PE from ICU Ward of Tianjin Infectious Diseases Hospital.Using three plasma components separators with different membrane pore (EC30W:Φ20-25nm:EC40W:Φ25-35nm: EC50W:Φ35-37nm) to filtrate1000mL plasma and taking3mL plasma before filtration, recycled liquid and discarded liquid respectively.Quantitative PCR were used to examine HBV DNA in Sera. Quantification of the HBsAg. HBeAg in sera was quantified using electrochemical Chemiluminescence immunoassay. Serum ALB and GLB were detected by Toshiba120automatic biochemical analytical instruments, to elect the plasma components separator suitable for this experiment. Then the elected plasma components separator filtrate2500mL plasma, total circulating filtration5000mL plasma, taking3mL plasma before filtration, recycled liquid and discarded liquid respectively filtrating1000mL plasma to used to detect HBV-DNA.HBsAg. ALB and GLB. in order to understand the relationship between different amount of circulating plasma and the clearance of Hepatitis B Virus.Results1. Using EC30W, EC40W. EC50W three kinds of plasma components separator filtrate1000mL plasma respectively. In plasma before filtration HBV-DNA is3.0×105copies/mL. HBsAg content is5102COI. In recycled liquid HBV-DNA is undetectable, HBsAg levels also is decreased significantly, the filtration rate of HBV-DNA and HBsAg of three plasma components separator is close to0.2. Using EC30W, EC40W, EC50W three kinds of plasma components separator filtrate1000mL plasma respectively, in recycled liquid HBsAg content is1.1COI,1.18COI,34.64COI separately. After using EC50W plasma component separator to filtrate, the HBsAg content is increasing significantly than it by using EC30W and EC40W plasma component separator.3. Using EC30W, EC40W, EC50W three kinds of plasma components separator filtrate1000mL plasma respectively. In plasma before filtration ALB is22.2g/L GLB is17.2g/L. In recycled liquid ALB is14.5g/L、17g/Land20.6g/L respectively, GLB is4.3g/L、7.4g/L and12.3g/L separately.4. After filtrating circularly by using EC40W plasma components separator, the clearance rates of HBV-DNA are respective66.22%,74.10%.81.42%.92.84%and94.37%;the clearance rates of HBsAg are45.39%,64.31%,80.45%,88.27%and90.57%;the loss rates of ALB are23.61%,25.23%,22.49%,32.44%and34.64%; the loss rates of GLB are45.93%,43.95%,43.56%,58.00%and61.38%.Conclusion1.After filtrating by using three kinds of plasma component separator with different membrane pore, in recycled liquid HBV-DNA and HBsAg are decreased significantly, but with the increase of membrane pore, the content of HBsAg in recycled liquid is also increasing, which is relationship with the membrane pore of plasma component separator and the size of HBV-DNA and HBsAg.2. With the increase of membrane pore, the content of ALB and GLB in recycled liquid are also increased significantly. Because the molecular weight of ALB is bigger than it of GLB, the ALB filtration rate of plasma components separator is more increasing than it of GLB.3. When using EC40W plasma components separator, with the increase of amount of circulating plasma, the clearance of HBV-DNA and HBsAg are also increased, but for the high load of HBV DNA and HBsAg in the plasma, in order to make down to the desired level, many times of DFPP treatment may be needed.4. When using EC40W plasma components separator, with the increase of amount of circulating plasma gradually, the loss rate of ALB and GLB are basically stable, so after filtrating by Using the EC40W plasma component separator, not only Hepatitis B Virus are cleared, but also the loss of plasma proteins are reduced...
