Background and Objective:CG (Cystitis glandularis) is a disease with hyperplasia and metaplasia in mucous membrance of urinary bladder. It is a benign disease, but there is some probability to have malignant change. So it is in precancerous lesion. The pathogenesis and canceration are not clear now. The researches of the disease focus on diagnosis and treatment, the basis researches are less. Recently the relationship of nonresolving inflammation and tumor is a international hot spot, especially the mechanism of nonresolving inflammation changing to canceration. In this research, we collected clinical data of CG patients and followed up; transmission electron microscope was used to observe the ultra-structures of CG tissues. We intend to investigate the role of nonresolving inflammation in the pathogenesis and progression of CG. Examine the expression of Interleukin-6, Interleukin-17and TGF-P in CG tissues from the direction of molecular pathology. Approach the role of nonresolving inflammation in CG.Materials and Method:A total of56patients of CG from department of urology, second hospital of Tianjin medical university undergoing surgical resection or TURBT treatment, with accession from December2006to December2010, were included in this study. A total of15patients of CG from department of urology, second hospital of Tianjin medical university undergoing surgical resection or TURBT treatment, with accession from Mar2011to Mar2012, were collected and observed in the transmission electron microscope, summarize clinical and pathological feature of CG.43specimens of curative cystitis glandularis as Group B;13specimens of cystitis glandularis as Group C. Collect15specimens of normal mucous membrane of bladder as Group A. Strept Actividin-Biotin Complex (SABC) method was used to detect the expression of IL-6, IL-17and TGF-β in CG tissues.Staining steps were carried out according to kit instructions, the results were observed by light microscope, and buffy particles in cells were considered as positive cases, brown as strong positive. SPSS17.0software application was used for all analysis and statistical significance was defined as P values less than0.05.Results:In the observation of the transmission electron microscope, the degeneration of transitional epithelium was found, there were many vacuoles in the cells, the connection of cells was lax; there were many lumens of gland under the mucous membrane; there were plenty of mitochondrions in the gland cells, so as to rough endoplasmic reticulum and golgi body; many secretory granules in cytoplasm can be found, inflammatory cells surrounded the lumens of gland; interstitial edema and water space can be observed, so as to vessel walls’breakage. Positive rates of IL-6in Group A、B、C were33.3%(5/15)、13.3%(2/15)、20%(3/15) positive rates of IL-17in Group A、B、C were51.2%(22/43),65.1%(28/43),60.5%(26/43); positive rates of TGF-β in Group A、B、C were100%(12/13),100%(13/13),92.3%(12/13). In group A, the number of negative expression of3cytokines was9; the number of positive expression of only one cytokine was5; the number of positive expression of2or3cytokines was1. In group B, the number of negative expression of3cytokines was7; the number of positive expression of only one cytokine was19; the number of positive expression of2or3cytokines was17. In group C, the number of negative expression of3cytokines was0; the number of positive expression of only one cytokine was1; the number of positive expression of2or3cytokines was12. There were significant differences between the3groups by χ2test (P<0.01); positive correlation exited between IL-6, IL-17,TGF-β and formation and carcinomatous change of CG (P<0.05); positive correlation existed between IL-17and IL-6,TGF-(3in the result of spearman rank correlation test (P<0.05). There were significant differences of combined expression of3cytokines between the3groups (P<0.01).Conclusion:Inflammatory cells aggregated and vessel walls’breakage were observed in transmission electron microscope; IL-6、IL-17and TGF-β showed high expression rates in CG tissues, the expression rates were higher with recurrence and canceration, positive correlation existed between the3cytokines. All of these indicated nonresolving inflammation may play an important role in the pathogenesis and progression of CG.
