Object: To investigate the effects of prolonged pneumoperitoneum on renalfunction.And with isoflurane for comparision,observe the protective effects ofulinastatin or propofol combined with isoflurane undergoing laparoscopic surgery.Methods:60patients undergone electived laparoscopic operation ofabdominal region in Guangxi medical university first affiliated hospital of the westin March2011to November. They were randomly divied into three groups, A, B,and C(n=20). Group A accepted isoflurane,group B accepted isoflurane andulinastatin, group C accepted isoflurane and propofol after induction. MAP, HR,Hb, BUN, Cr, blood gas analysis, the quantity of transfusion and hemorrhage andurine were monitored before anesthesia(T0), at180min(T1),240min(T2) afterinsufflation, at60min(T3) after operation, on postoperative days1(T4),2(T5), and3(T6).Results: There was no significant difference in amount of intraoperative fluidadministered,amount of bleeding and urinary output between three groups. Ingroup A, BUN and Cr were increased significantly from T1to T6compared withpreanesthesia. In groups B and C, Cr and BUN were increased significantly fromT2to T5, while lower than those into pneumoperitoneum and postoperative ingroup A, but there were not significant while comparing with each other.Conclution: The damage of CO2pneumoperitoneum on renal function was increased with the extension of time,and could not recovered soon postoperative.The protective effects of ulinastatin or propofol combined with isoflurane on renalfunction were better than using isoflurane alone.
