| Objective:The experimental study of ischemia modified albumin (IMA) in patients with acute pancreatitis in vivo changes in level, and to explore its and triglycerides, malondialdehyde, endothelin, C-reactive protein and whether a correlation exists, whether IMA may act as pancreatic microcirculatory disturbance in a bless card and assessment of severity of acute pancreatitis in a simple and easy by means of. Methods:The inclusion criteria and diagnostic criteria for identification of35patients after onset in48hours in patients with acute pancreatitis as case group. During the same period,20cases as the control group of healthy subjects. Record case group and control group, gender, age and albumin level. The case group on admission after the APACHEII score and CT score. Collected blood samples in before treatment,3days after treatment,7days after treatment, and blood samples of control group. Serum specimens and plasma centrifuged from blood samples respectively, were stored in-70℃. Simultaneous detection of cases of the same time period C reactive protein. According to its prognosis, case group divided into severe and mild group, severe group includes the patients who had occurred complications, local complications, death and surgical case. In comparison to the control group, SAP group and MAP group IMA, endothelin, MDA values. Results:1.Control group, MAP group and SAP group age composition between the three groups, gender, albumin levels were not statistically different (P>0.05).2. The level of IMA in the control group, is less than MAP group and SAP group (P<0.05). Before beginning treatment, the difference of IMA between MAP group and SAP group were significantly, after accepting3days’hospitalization treatment, the level of IMA in MAP group decreased significantly(P<0.05. vs control group), while, after7days’hospitalization treatment, the level of IMA in MAP group and SAP group was not changed significantly(P<0.05. vs control group).3. ET:The levels of endothelin in SAP group was significantly higher than the MAP group and control group (P<0.05); after3days’hospitalization treatment, the decreasing of ET in SAP group was not significantly, the difference between SAP group and MAP group is statistically significant(P<0.05). Moreover, after7days’hospitalization treatment, it was not decreased significantly, too(P<0.05. vs MAP group).4. Before hospitalization treatment, the level of MDA in control group was lower than MAP group and SAP group. After3days’hospitalization treatment, the level of MDA in MAP group was decreased significantly, the difference between MAP and SAP group statistically significant (P<0.05). While, after7days hospitalization treatment, the decreasing of MDA in SAP group was not significantly (P<0.05. vs MAP group).5. CRP Before hospitalization treatment, after3days and7days hospitalization treatment, the level of CRP in control group was lower than MAP group and SAP group (P<0.05). The CRP level of MAP is lower than SAP in the all-time(P<0.05). Moreover, after hospitalization treatment, the decreasing of CRP in SAP group was significantly, the difference between MAP group and SAP group is statistically significantly(P<0.05). Conclusion:1. IMA may be elevated in acute pancreatitis, the IMA may be used as one of the microcirculation of evidence of pancreatitis.2. The body of endothelin in the SAP group sustained a high level, it played an important pathophysiological role in the development process in SAP occurred.3. Malondialdehyde may be more stable to reflect the body affected by the severity of the attack from free radicals, changes in the dynamic observation of the MDA can be used as one of the indicators to assess the severity of acute pancreatitis and prognosis.4. Changes in the dynamic observation of CRP can be used as one of the indicators to assess the prognosis of acute pancreatitis. |
