Abstract:Roux-en-Y gastric bypass surgery is a new method of curing type-2diabetes with a high efficiency. But, up to now, its mechanism is not totally clear to us. Insulitis and inflammatory in blood circulation are the important factors of the development of type-2diabetes. Inflammatory in circulation such as IL-1β, TNF-α will aggravate insulin resistance in T2DM patients; and the insulitis will aggravate beta-cell damage and mediate beta-cell apoptosis, and finally lead to the fibration of islets of langerhans and to the happening and aggravation of diabetes.Objective:The purpose of the present experiment is to observe gastric bypass surgery’s effect on the inflammatory reaction of GK rat, discuss the potential mechanism to curing diabetes of gastric bypass surgery.GK rat was used as experimental diabetic animal model, SPF-class healthy male GK rats were randomly divided into two groups:20for operation group and20for sham operation group. Carry out gastric bypass surgery to the operation group, and transection and reanastomosis of the jejunum tract to the sham operation group. Measure the weight and fasting glucose of the two groups of rats respectively before operation and1,2and4weeks after operation, and do the OGTT experiment to them. Before and4weeks after operation, detect insulin expression in serum of each groups’ rats by radioactive immunoassay, calculate the numbers of IRI and IAI. Before operation and4weeks after operation detect TNF-α, IL-1β. CRP and IL-6expression in serum by ELISA, and detect the expression of insulin, TNF-a, IL-1β, macrophages, FN and CO-III in islet by immunofluorescence double stain and histochemistry. Assay the level of the expression of insulin, L-β and TNF-a in islet by Image-Pro plus6.0image analyze software.1. Compared to the sham operation group, the weight of the rat was markedly decreased1,2and4weeks after operation, and the operation group’s rats’fasting glucose and OGTT2h glucose was markedly decreased4weeks after operation. The difference was statistically significant (p<0.05).2. Compared to the sham operation group, the operation group’s rats’insulin expression level in serum4weeks after operation was not significantly different ((p≥0.05). Compared to the sham operation group, the operation group’s rats’ number of IRI was markedly decreased and number of IAI was markedly elevated4weeks after operation, the difference was statistically significant (p<0.05).3, Compared to pre-operation and the sham operation group, the operation group’s rats’ TNF-α、IL-1β、CRP、IL-6expression level in serum was markedly decreased4weeks after operation, the difference was statistically significant (p<0.05).4, Compared to the sham operation group, the operation group’s rats’TNF-α、IL-1β expression level in islet was markedly decreased4weeks after operation, and insulin in islet was markedly elevated, the difference was statistically significant (p<0.05).5, Compared to the sham operation group, the operation group’s rats’ macrophages, FN and CO-Ⅲ expression level in islet was markedly decreased4weeks after operation, the difference was statistically significant (p<0.05).1, Gastric bypass surgery can control the weight and blood glucose of type-2diabetic rats well, improve the insulin resistance and enhance insulin sensibility of diabetic rats, and finally improve the type-2diabetes. When the weight of the rat recovered, the surgery still control blood glucose well.2, Gastric bypass surgery can decrease the expression of inflammatory in serum, and improve the inflammatory reaction of diabetic rats, which may be one of the factors of the improvement of insulin resistance of diabetic rat after gastric bypass surgery.3, Gastric bypass surgery can decrease the inflammatory reaction in islets of langerhans of type-2diabetic rat, and improve the function of beta-cell in islet, which may be one of the mechanisms of the level of insulin increased in islet, and the improvement of blood glucose after gastric bypass surgery.4, Gastric bypass surgery can decrease the fibration degree in islets of langerhans of type-2diabetic rat, decrease the expression of macrophages in islet, which would prove gastric bypass surgery can improve the pathological damage in islet, and alleviate its development in morphology.
