Objective: To observe the tissue pathology and structurechange in human saccular intracranial aneurysms. To detect theexpression of CHRM5in human cerebal saccular aneurysm andexplore the role in the pathogenesis of intracranial aneurysmsoccurrence, development and rupture.Methods: samples are divided into case group and control groups.40surgical specimens of human cerebral aneurysm from17males and23females in the Attach Hospital of Luzhou Medical College during2010.5~2012.2were choosen.All specimens were got fromdissecting after clipping the aneurysm;40control specimensincluded27cases of suferficial artery,8anterior cerebral artery and5middle cerebral artery..These specimens came from hematomaremoval and internal decompression.To observe the all specimenswith macroscopicAnd light microscope.Slice preparation for HE dyeing andimmunohistochemical method to detect the expression of CHRM5in two sets of specimens,Compare the the differences between them.Results:1macroscopic observation Most of the specimens werepurple,dark blue or brown.Atherosclerosis plaques could be seenin some of the aneurismal specimens.There were thrombosis insome cavities of the aneurysms.The wall of the aneurysm varied,the bottom Was thicker than the top.Sometimes,only a thinmembrane at the top of the aneurysm was left,some wereruptured.2.H&E staining:The fundamental pathological changein cerebal aneurysm wall was the number of endothelial celldecrease;disintegration of internal of elastic layer were scarce inthe tunica media and displaced by fiber tissue;the outer layerturned thinner,most was fibre tissue,no outer elastic layer.In all layers of aneurysm were coated with more or less inflammationcells,regional calcification,thrombosis and different stage ofatherosclerosis in the aneurysm wall.3.Immunohistochemistrystain:The positive expression of CHRM5in some samples in thewall of aneurysm and normal artery,It was mainly expressed in theinner and middle layer of aneurysm and endothelial cell,smoothmuscle cell and macrophage cell.Positive expression of CHRM5inthe wall of intracranial aneurysm shows a clay bank staining of thenucleus and cytoplasm.Compared to the control groups,theexpression of CHRM5in the aneurysm wall of aneurysm is loweven no expression, the positive differences in the two groups wasstatistically (P<0.01), and case CHRM5positive expression thancontrol groups arterial walls weakened significantly. CHRM5withinthe normal artery wall, middle layer, inner layer and outer layerpositive expression rate reduce, compared the differences betweenevery2layer,they are statistically significant (P<0.05). butCHRM5in the wall of the saccular aneurysms, compared thepositive expression rate of three layer, the differences were notstatistically significant (P>0.05)each other.Conclusions:1.The morphological and pathological characters ofhuman intracranial aneurysm were inflammation and its destructionof the aneurismal wall. The main inflammatory cell weremacrophage and lymphatic cells.2. Compared to the controlgroups, the expression of CHRM5in the aneurysm wall ofaneurysm is low even no expression, Speculated that CHRM5maybe participate in the pathogenesis of formation and rupture ofaneurysm.3. The smooth muscle cells in aneurysm wall reductionobviously, the reducing of CHRM5may results in impairedfunction of endothelial cells, stimulate inflammation andatherosclerosis, vascular smooth muscle cells proliferation andapoptosis caused by imbalances; also there may be a film in neuralbirth defects at their disposal, make smooth muscle cell growthdisorders, or caused by nerve impulses reduce stress factors such aschanges in blood-related.4.The reducing of CHRM5may be involved in pathological changes of cerebral Vasospasm afteraneurysm rupture, influence cognitive function.5. The researchprovided new experimental basis for prevention and treatment ofintracranial aneurysm on the molecular level.
