ObjectiveAnalyse the pathogenesis, clinical features treatment strategies and efficacy of SAP in our hospital. At the same time, explore the value of APACHE II score. Ranson score and Balthazar CT grading on the SAP disease severity and prognosis.MethodsRetrospectively analyse of110SAP cases, admitted in Xiangya hospital from January2005to January2010.ResultsOf al1the110patients,77were induced by biliary diseases, while16were induced by alcohol,15were induced by hyperlipemia and19were induced by others. The main clinical manifestation of SAP is areola.Higher the score of APACHE II Ranson score and Balthazar CT grading is, higher the mortality rate will be. Balthazar CT grading has the highest sensitivity.88cases of110SAP patients were cured, and22cases (20.0%) were dead.75patients were with non-surgical treatment,62patients were cured and13died (17.3%);22cases had early surgical treatment,15cases were cured,7died (31.8%); in the late surgery in13cases,11cases were cured,2died (15.4%).Early deaths were mainly from MODS (13patients,76.5%), and the later reason of deaths was severe secondary infection (5cases,100.0%).ConclusionsIn our hospital, the major couse of SAP is biliary diseases, and the secondary cause is alcohol.Higher the score of APACHE II Ranson score and Balthazar CT grading is, higher the mortality rate will be. Balthazar CT grading has the highest sensitivity.The mortality of SAP in our Hospital is similar to international level.The main cause of death at an early stage is MODS, and the late reason of death is serious infection.
