Objective To explore the Relationship between R219K polymorphism of ATP-binding cassette transporter A1R219K and different sub-type of OCSP in cerebral infarction and the Relationship between R219K polymorphism and type2diabetes mellitus. Methods A case-control study on351patients with cerebral infarction and279healthy controls was carried out. The genotype and allele frequencies of ABC A1R219K gene polymorphism were assayed by polymerase chain reaction-restricted fragments length polymorphism (PCR-RFLP).Results BS, TG TC, LDL-C and CREA level of the Cerebral infarction group is higher than those of the control group, but the level of CREA is in the normal clinical range;TG level of the different sub-type of OCSP in cerebral infarction is higher than that of the control group. HDL-C level of the Cerebral infarction group is significantly lower than that of the control group, HDL-C level of the different sub-type of OCSP in cerebral infarction is lower than that of the control group.the level of HDL-Cå'ŒApoA-1of the KK Genotype group are significantly higher than those of the RK and RR genotype group. There were no statistically significant for ABCA1R219K genotype frequencies between cerebral infarction and the healthy controls(P>0.05). There were no statistically significant for ABCA1R219K genotype frequencies between the different cerebral infarction of OCSP(P>0.05). The risk factors for cerebral infarction, include family history of stroke, hypertension, drinking history, blood glucose, HDL-C levels, HDL-C which is a protective factor. There were no statistically significant for ABCA1R219K genotype frequencies between the cerebral infarction with type2diabetes mellitus and the cerebral infarction with no type2diabetes mellitus (P>0.05).Conclusion:1. ABCA1219K homozygotes may be related to beneficial lipid profile, HLD-C may be a protective factor of cerebral infarction.2. There is no statistically significant for ABCA1R219K genotype frequencies between the patients with cerebral infarction and the healthy controls. There is no statistically significant for ABCA1R219K genotype frequencies between the patients with cerebral infarction in OCSP and the healthy controls.3. There were no statistically significant for ABCA1R219K genotype frequencies between the cerebral infarction with type2diabetes mellitus and the cerebral infarction with no type2diabetes mellitus.
