Objective:To investigate the incidence of PCOS, the correlative factors for causing PCOS,clinic features of PCOS who are in reproductive age woman in ChangSha,and to explore the related factors of insulin resistance of poly cystic ovary syndrome.Methods:We investigated1794reproductive women who are among19-45year old from the community of ChangSha city(include4community and1country) by using the cluster sampling means from2009-2010.The investigation included investigative questionares,physical exam,ultrasonic scanning,collecting blood samples,the the use of Chemiluminescence Method for the determinination serum hormone and fasting insulin levels;the use of selective removal method for the determination of triglycerides, high density lipoprotein.We had screened149doubtful PCOS females according to the diagnositic criteria of Rotterdam in2003(including that cronic anovulia or oligotrichia,high androgen in clinic/hyperandrogenism),the number of2-9mm antro-follicle that is equal to or more than12.If the female meets one of the four factors,she will be equal to or more than12.If the female meets one of the four factors,she will be considered as the doubtful PCOS candidate).The control group was177.The multiple factors Logistic regression analysis were applied for IR of PCOS.Results:(1) According to the Rotterdam2003Critieria,130PCOS patients were diagnosed in1488subjects.the prevalence of PCOS in Changsha city was8.73%.(2) Among all PCOS patients, oligo-ovulation was the most common clinical manifestations(88.46%),followed by polycystic ovary PCO (82.31%), and hyperandrogenism (47.69%); In the age of menarche, BMI, WHR, Blood pressure,the difference was no significantly between PCOS and control group; In a family history of hypertension, mother with oligomenorrhea, infertility, and fathers early bald, although there was a higher tendency in PCOS, the difference was not significant;With the corresponding control group, FAI, SHBG, FBS were significantly different.(P<0.05).(3) By China Obesity critieria,PCOS patients were divided into3type(BMI<18.5kg/m2,BMI18.5kg/m2~23.9kg/m2,BMI≥24.0kg/m2),PCO S patients, serology, markers were significant different(P<0.05).Patients with BMI<18.5kg/m2, SHBG was higher than other groups.FAI,FINS,IR were lower than other group, the difference was statistically significant, there was no significant difference between different groups in FBS and blood lipids; (4) There was no significant difference among PCOS patients of different ages in serum markers (P>0.05).(5)In PCOS patients, IR was negatively correlated with SHBG (r=-0.964, P=0.011<0.05).Conclusions:(1) Acordding to the2003Rotterdam criterias, the prevalence of PCOS in Changsha city was8.73%.(2) Oligo-ovulation is the top clinical manifestation of Changsha woman with PCOS, with a decrease in SHBG and an increase in FAI and FBS in serum markers, with significant difference (P<0.05);(3) The PCOS patients had lower hyperandrogenism and insulin resistance when BMI were lower than18.5kg/m2.(4) There was no significant difference among PCOS patients of different ages in serum change.(5) Attention should be paid to IR detection in PCOS patients who had hyperandrogenism.
