Object Using Voxel-based analysis of MRI, we examined the different density and volume of gray matter in patients with temporal lobe epilepsy (TLE), in order to investigate the abnormality of temporal lobe and out of temporal lobe.Methods42patients with temporal lobe epilepsy and30gender, age and education-matched healthy controls were underwent3.0T MRI scan and collected three-dimensional magnetization prepared rapid acquisition gradient echo sequences (T1WI-3D-MPRAGE) datasets of the whole brain. Voxel-based morphometry (VBM) was employed to analyse the different density and volume of gray matter between controls and patients.Results42patients with temporal lobe epilepsy and30gender, age and education-matched healthy controls finished MRI scan.Cases were excluded because of head movement. Finally,30patients (13males/17females) and30healthy controls (16males/14females) were entered analysis.We found that (1) Compared with healthy controls, patients with TLE showed significantly decreased the density of gray matter in left occipital lobe and cuneate lobe, right Culmen; while density of gray matter in bilateral Putamen, Lateral Globus Pallidus, left inferior temporary gyrus,and right inferior frontal gyrus was significantly increased in patients.(2) Compared with healthy controls, patients with TLE showed significantly decreased the volume of gray matter in bilateral insular lobe, right Precentral Gyrus, superior temporal gyrus, cuneus, Precuneus,Middle Occipital Gyrus and supramarginal gyrus, left Middle Temporal Gyrus, Lingual Gyrus, angular gyrus, transverse temporal gyrus,posterior central gyrus,paracentral gyrus, Parahippoca- mpal Gyrus and Fusifonn Gyrus; while no region with the volume of gray matter was significantly increased in patients.Conclusions1.In temporal lobe epilepsy (TLE), abnormalities of gray matter were not just seen in the temporal lobe, but also seen in other lobe. That suggested cryptogenic TLE might exist a special impainnent of neuronal network, which played a major role in the pathogenesis of TLE.2.Wecould detect the extensive abnormalities of the brain structure by VBM in patients with routine MRI-negative.
