Objective: To explore the mechanisms and provide theoretical basis for clinical useof Fu xin decoction through the observation of the decoction on Adriamycin (Adriamycin,ADR)-induced effect of myocardial cell apoptosis in CHF rats,and to observe clinicalcurative effect of Fu xin decoction on patients with CHF.Methods:Basic research:Established heart failure rat model induced by ADR.60male Wister rats, randomly divided into5groups, blank-control group, model group, Fuxin decoction in the low-dose group, Fu xin decoction in the high-dose group, the captoprilgroup, per set of12. After6weeks of drug intervention, determining the success of modelthrough the detection of cardiac function. Semi-quantitative analysis on myocardialCaspase-8mRNA and Bcl-2mRNA by RT-PCR. Clinical observation:Randomly selected64cases of inpatients met the inclusion criteria of CHF, into treatment group(32cases)andcontrol group(32cases). Control group treated by conventional Western medicine,treatment group in Fu xin decoction rushing on the basis of conventional Western medicineEffects were observed after2~4weeks’ treatment.Results: Basic research:The model group rats show obvious symptoms of heartfailure. The symptom of the other treatment groups is lighter than the model group.Compared with the model group, the other treatment groups upregulated its Caspase-8,Bcl-2downregulated significantly. Clinical observation:After the course of treatment,significant difference was apparent in the clinical effects:In the treatment group,the totaleffective rate is87.50%; in the control group,the total effective rate is78.13%,the curativeeffect of the treatment group is clearly superior than the control group(P<0.05). In terms ofefficacy indicators, treatment group was better than the control group in cardiac function (P<0.05),EF、FS、SV and BNP (P<0.05).Conclusion: Basic research:1. Fuxin decoction can significantly improve theaverage peak left ventricular pressure in rats with congestive heart failure(LVSP),anaverage of ventricular diastolic pressure(LVDP)and diastolic pressure differential(±DP/dtmax),enhanced cardiac function.2. Fuxin decoction can significantly reduce the BNPlevel in plasma and myocardium of rats with congestive heart failure.3. Fu Xin DecoctionCan inhibit myocardial cells apoptosis by Upregulating the Caspase-8expression anddownregulating the Bcl-2expression which is the mechanism of anti-heart failure of FuXin Decoction. Clinical observation:Fu Xin Decoction Can effectively ameliorate CHFpatient’s Indexes of heart function such as EF、FS、SV, lower their BNP in Plasma andenhance cardiac function.
