Objective:This thesis is to analyze the clinical objective data of patients with primarytrigeminal neuralgia,study the relevant factors affecting its incidence by the melhod ofepidemiology,and treat the patients belonging to syndrome of wind-pathogen invasion withXiFengZhiTong decoction to provide data support for clinical entervention.Methods:1.Combining clinical experience with relevant references on trigeminalneuralgia,we make Cases Form for clinical observation,and then perform statisticalanalysis by collecting them from patients,so as to explore the distributing features of thissyndrome,as well as its relation with such factors as gender,age,emotion,life style.At lastWe summarize the relative elements affecting primary trigeminal neuralgia.2.We treat43of the patients belonging to syndrome of wind-fire stasis withXiFengZhiTong decoction for one month, grade their pain degree,seizure frequency,sympt-om score before treatment,after two weeks’ and four weeks’ treatment,pay follow-up visitsafter three months’,and anylize the results by statistical software SPSS17.0.Results:1. Symptomatology study of TCM(1)According to their pain character,the top three distributions in the120cases are:burning at sore spot (36.7%),both burning at sore spot and sting pain (17.5%),and dullpain(13.3%).Commonly concomitant symptoms are anorexia(79.2%),anemophobia(43.3%)thirsty(36.7%),bittertaste(35%), astriction(29.2%), insomnia and dreaminess(27.5%),redd-ened face and eyes (27.5%),kakostomia(20.8%), vertigo(20%),rough skin. Comprehensiveclinical syndrome distribution investigation results show that syndrome of wind-fire stasisaccounts for35.8%,stomach-fire rising is30.8%,wind-pathogen invasion is9.2%,yang hyperactivity and yin deficiency disorders is9.2%, wind-phlegm obstruction is7.5%, anddeficiency in qi and blood is7.5%.(2)Morbidity of male and female is43.3%and56.7%respectively, Females is morethan males. syndrome of stomach-fire rising accounts more in male(15.8%), syndrome ofwind-fire stasis and syndrome of stomach-fire rising account25.8%、15.0%respectly. InThe age distribution of patients, middle aged group accounts for the most(73.4%),majorityare syndrome of stomach-fire (28.3%) and wind-fire stasis (32.5%).Junior group register assyndrome of stomach-fire (1.6%) and wind-fire stasis (1.6%).Presenile group register assyndrome of wind-phlegm(7.5%) yin deficiency disorders(7.5%) and syndrome of thedeficiency in qi and blood(5.0%). The survey also found that upset irritability and foodspicy connect with it.In addition, the investigation confirmed that patients can be inducedby a variety of factors, which touch on the face caused by93.3%, Emotional distress, bythe wind, the proportion of work accounts for35%、43.3%、25%respectively.2. Clinical observation:(1)Short-term clinical efficacy: treatment of the2nd week and4th week of totaleffective rate of clinical is81.4%and90.7%respectively, effective rate symptoms is72.1%and95.3%respectively.As the treatment of extension of time,there are significantdifferences in TCM symptom score (P<0.05).(2)Long term clinical effects:3-month follow-up compared with the treatment of the2nd week, clinical and therapeutic effect of TCM symptom p <0.05to significantdifferences;4th week3-month follow-up and treatment, clinical and therapeutic effect ofTCM symptom p>0.05, no significant differences.Conclusions:The occurrence and evolution of primajy trigeminal neuralgia closelyrelated to human ’s liver and stomach,the syndrome of which is manifested clinically bywind-fire stasis and stomach-fire rising caused by wind,fire and stasis.The preliminaryobservation of XiFengZhiTong decoction therapy is indicated for the treatment of primarytrigeminal neuralgia.Knowledge about the wholesome life should be popularized amongprimary trigeminal neuralgia patients:keep regular hours,avoid over-fatigue,stay warm,keepa light diet,live in pea ceful mood and do enough exercise,which is especially true forthose female patients over middle age and easy to be impatient and irritable.
