Objective:To study the influence of Yunnan Baiyao in perioperative bleeding and changes of platelet activity in mitral valve replacement with cardiopulmonary bypass (CPB).Methods:Twenty-five selective patients undergoing mitral valve replacement with cardiopulmonary bypass were randomly assigned to two groups:Yunnan Baiyao group (n=10) and control group (n=15). Three days before the operation, patients in Yunan Baiyao group started taking0.5g (2capsules) of Yunan Baiyao each time,4times daily for three consecutive days. We detected HB, RBC, HCT, PLC, APTT, PT, ACT, D-dimer, GP-ⅡbⅢa and GMP-140after the admission (baseline,T0), before skin incision (T1),30minutes after CPB was started (T2),20minutes after protamine administration (T3),24hours after operation (T4) respectively. And we recorded the cumulative drainage volume of8h,8-24h and24h postoperatively. and the average number of packed red blood cell units transfused intraoperatively and postoperatively.Results:1.For patients in Yunnan Baiyao group, there was a trend toward lower cumulative drainage volume of8h.8~24h and24h postoperatively, and less packed red blood cell units transfused intraoperatively and postoperatively compared with those in control group, but the differences were not statistically significant (all P>0.05).2.The FG level was significantly lower in Yunnan Baiyao group at T4compared to control group (P<0.01); the PT level was significantly shorter in Yunnan Baiyao group at T4compared to control group (P<0.05):there were no statistical differences between the two groups in ACT at all time points. RBC, PLC. TT and APTT at T4(all P>0.05).3. There were no statistical differences in PLC, GP-ⅡbⅢa, GMP-140at T1compared with baseline (all P>0.05).Conclusion:Yunnan Baiyao has the trend to reduce the average number of packed red blood cell units and cumulative drainage volume perioperatively, has positive effect on extrinsic coagulation pathway, reduce postoperative hypercoagulation and has no adverse effects.
